Interaction between carbamazepine and fluvoxamine

Abstract: In 3 patients the addition of fluvoxamine to a constant dosage of carbamazepine (CZP) caused a substantial rise of plasma CZP accompanied by symptoms of intoxication. As this drug combination may occur increasingly in the future, this probably pharmacokinetic interaction is of practical relevance.

“…Specifically, several ADs of the SSRI family are inhibitors of several isoenzymes of the cytochrome P450 system. These include fluoxetine, paroxetine, and fluvoxamine (107)(108)(109). Sertraline and citalopram are less likely to have pharmacokinetic interactions with AEDs (71).…”

“…CBZ is metabolized by the P450 3A isoenzymes (244,246), which are believed to be inhibited by fluoxetine, fluvoxamine, sertraline, and nefazodone (244). A normal volunteer study demonstrated elevated CBZ levels after concomitant medication with fluoxetine (254), and adverse events have been reported, with raised plasma concentrations of CBZ, with fluoxetine (255,256), fluvoxamine (257,258), sertraline (259), and nefazodone (260). Raised levels have also been reported with viloxazine (261).…”

“…Drug interactions that increase the concentrations of carbamazepine or carbamazepine 10,11-epoxide have the potential to exacerbate seizures. Among psychotropic agents, viloxazine [124], trazodone [125], fluoxetine [126] and fluvoxamine [127] have been reported to increase carbamazepine concentrations. Newer antipsychotic agents are weak inhibitors of CYP450 enzymes and are unlikely to have a significant effect on carbamazepine levels.…”
