Inter-reader variability in chest radiography and HRCT for the early detection of asbestos-related lung and pleural abnormalities in a cohort of 636 asbestos-exposed subjects

Abstract: High resolution computed tomography scans are superior to X-rays in detecting lung alterations after asbestos exposure and are supposedly easier to interpret. Nevertheless, inter-observer variability did not differ between the two methods in this study. This was probably due to the only discrete asbestos-related lung or pleura alterations of this cohort and to the unfamiliar CT classification sheet, which was revised on the basis of the presented results.

“…As the prevalence of lung cancer is said to correlate with asbestosis, early detection of asbestosis related lung diseases may improve the prognosis and chances of survival for subjects exposed to asbestos 22,23) . In a survey conducted at an asbestos composite factory in Mumbai, India, Murlidhar and Kandhere stressed the lack of training of doctors in the diagnosis of occupational diseases as the main factor leading to the misdiagnosis of asbestosis as either chronic bronchitis or tuberculosis 24) .…”

“…The algorithm was also tested on image data from two real different points in time and yields plausible results. Due to the high inter-and intra-reader variability [10] in quantification, these results are not presented.…”

“…Evaluating the presented method for non-simulated growth is not easily possible, as the manual diagnosis is subject to intraand inter-reader variability [10]. Therefore, we evaluate the presented method by generating artificial growth, or to be exact we artificially shrink a thickening from the second point in time (t = 1).…”

“…CT scans were interpreted independently by two experienced radiologists using previously described criteria. 30 Any lack of concordance in the interpretation was resolved by consensus. Tuberculin skin tests (TSTs), using monopuncture intradermal technique (Mantoux): TSTs were performed in workers with evidence of pleural (circumscribed (plaques) or diffuse thickening or calcification) or parenchymal abnormalities.…”
