Effect of a Two-hour Training on Physicians' Skill in Interpreting PneumoconioticChest Radiographs: Nlandu Roger NGATU, et al. Department of Environmental Medicine, KochiMedical School, Kochi University-Objective: Occupational lung diseases have specific radiographic manifestations not always well known by physicians. In Japan, asbestos-related diseases became a public health concern after the "Kubota Shock", when a number of workers and residents living nearby a manufacturer of asbestos-made ducts developed mesothelioma caused by asbestos exposure. This preliminary intervention trial evaluated the effect of twohour training on inexperienced physicians' skill in interpreting pneumoconiotic chest radiographs. Methods: One hundred-two Japanese physicians participated in two reading-tests, using 12 radiographs, before and after the two-hour training with ILO/ICRP and Japan Pneumoconioses Study Group (JPSG) reading materials. Physicians had to check for the presence or absence of small opacity and pleural plaque consistent with pneumoconiosis. Sensitivity and specificity equal or greater than 70% were considered good, 50 to 69% acceptable and less than 50%, poor. Results: Post-training improvements in physicians' skills were seen. For small opacity, there was an increase in the proportion of physicians with good specificity, from 42% to 60%. For pleural plaque, the proportion of physicians with good specificity and good sensitivity increased, from 60% to 67% and from 18% to 25%, respectively. Also, significant improvements were observed in overall sensitivity for pleural plaque, from 46% to 60% (p<0.0001), and specificity for small Field Studyopacity, from 65% to 73% (p<0.0001). Conclusions: This study showed that two-hour participatory training may enhance physicians' skill in interpreting pneumoconiotic chest radiographs. There are countries without any pneumoconiosis screening program despite the WHO/ILO call for worldwide cooperation in eliminating it. Although the two-hour course cannot replace the five-day ILO workshop, such a program would be useful in areas with environmental or occupational exposure to dust. (J Occup Health 2010; 52: 294-301)

Pleural thickenings, which are an indicator for pleural mesothelioma, can be detected and analyzed using thoracic CTdata. They may cover a large area on the pleura surface and can have complex morphologies. Image noise, small deformations and especially the unclear boundary can significantly influence the segmentation process. Two independent segmentations from two points in time might independently suffer from these influences. As a consequence, they cannot be combined for reliable growth rate estimation. Therefore, we suggest a method to introduce consistency between both points in time. The method is carefully designed to estimate small growth from low resolution structures with complex morphologies. First, a surface mesh with segmentation confidence is extracted from one point in time. This mesh is refined considering the segmentation confidence and partial volume. Subsequently, this refined mesh is transferred to the other point in time and growth is tracked with sub-voxel precision.

Asbestos brake linings and blocks are currently used in heavy vehicle brake repair shops (BRSs) in Bogotá, Colombia. Some brake products are sold detached from their supports and without holes, requiring manipulation before installation. The aim of this study was to assess asbestos exposures and conduct a preliminary evaluation of respiratory health in workers of heavy vehicles in BRSs. To estimate asbestos exposures, personal and area samples were collected in two heavy vehicle BRSs. Each shop was sampled during six consecutive days for the entire work shift. Personal samples were collected on 10 workers including riveters, brake mechanics, and administrative staff. Among workers sampled, riveters had the highest phase contrast microscopy equivalent (PCME) asbestos concentrations, with 8-h time-weighted average (TWA) personal exposures ranging between 0.003 and 0.157 f/cm(3). Respiratory health evaluations were performed on the 10 workers sampled. Three workers (30%) had circumscribed pleural thickening (pleural plaques), with calcifications in two of them. This finding is strongly suggestive of asbestos exposure. The results of this study provide preliminary evidence that workers in heavy vehicle BRSs could be at excessive risk of developing asbestos-related diseases.
