Intentional Replantation: A Procedure as a Last Resort

Abstract: Aim: To address the indications, contraindications, surgical procedure, complications following treatment, factors influencing the results and success rate of intentional replantation procedure. Background: Intentional replantation is extraction of a tooth to do extraoral root canal therapy, curettage of apical lesion when present, and its replacement in its socket. It has been proposed as an alternative to routine extraction; however, it should be considered a last resort because the root may be fractured duri… Show more

“…Intentional replantation (surgical repositioning) [14] involves extraction of the extruded tooth, which is then reinserted into the socket as quickly as possible [15]. This procedure can lead to inflammatory root resorption, external root resorption, or replacement resorption [16][17][18][19][20][21][22][23][24]. Indeed, the method has several contraindications and should therefore be used only in selected cases [18].…”

Indeed, the method has several contraindications and should therefore be used only in selected cases [18]. A promising alternative is orthodontic repositioning [25][26][27][28][29], which involves moving the tooth back into its original position.…”

“…18 IR is described as extraction of a tooth for extraoral endodontic microsurgery and replantation of the tooth in its socket. 19,20 Endodontic microsurgery and IR offer opportunities to observe RCO directly under the microscope after removing the periapical soft tissues, resecting the roots and staining with methylene blue.…”
