Integration of Palliative Care Into All Serious Illness Care as A Human Right

Abstract: Deaths from COVID-19 are approaching 600 000 in the US and 3 million worldwide. As intensive care units have reached or exceeded capacity in many hospitals, concerns have grown about the ethical dimensions of rationed care. But many of these focused on who gets a ventilator, rather than how to provide palliative care to those who are seriously ill, including those treated with ventilators and those who may not want or cannot get a ventilator.

But evidence suggests that they lack confidence in their ability to do so.…”

A palliative approach can promote equitable health services and access to palliative care is considered a human right ( 30 ). With this approach dying is considered a normal process, but the World Health Organization (WHO) has estimated that only 14% of people receive palliative care when it is needed ( 31 , 32 ).…”

“…However, there is 1 phenomenon which is particularly prevalent in the setting of the COVID-19 pandemic, that is, an under-recognized risk for complicated grief: bereavement overload. 9 There is no simple definition of bereavement overload (BO), but it is a term that was coined decades ago to describe individuals' reactions to losses transpiring in a quick succession, without the time and opportunity for coping. 10,11 Grief overload, cumulative grief, and, more recently, "micro-losses" 12 have been used to describe this condition.…”

“…
The COVID-19 pandemic brought about bereavement overload as a risk factor for complicated grief. Bereavement overload (BO) describes individuals' reactions to losses transpiring in a quick succession, without the time and opportunity for coping [9]. It can occur during catastrophic events and impact everyone experiencing the loss.
