Background:Vertigo complaints are strongly associated with deterioration of patients’ quality of life (QoL). Vertigo and dizziness can result in depression and anxiety; therefore, the diagnosis and management of these concomitant psychiatric disorders can improve the patients’ quality of life. The aim of the present study was to contrast the QoL impairment and depression scores of patients suffering from different vestibular disorders. Methods:301 patients were examined due to vertiginous complaints at the Neurotologic Tertiary Rereferral Centre of the Department of Otolaryngology and Head and Neck Surgery of Semmelweis University. These patients completed the Hungarian version of the Dizziness Handicap Inventory (DHI), the Beck Depression Inventory, and the Symptom-Checklist-90-Revised (SCL-90-R) questionnaires. Results:According to neurotologic examination, the distribution of the different diagnoses was as follows: Ménière’s disease (n= 101), central vestibular disorders (n= 67), benign paroxysmal positional vertigo (n= 47), vestibular neuritis (n= 39), other unilateral peripheral vestibulopathy (n= 18), Persistent Postural-Perceptual Dizziness (n= 16), vestibular migraine (n= 8), and vestibular Schwannoma (n= 5). The results of the DHI questionnaire have indicated worsened QoL in 86.4%, out of which 33.6% was defined as severe. The Beck scale has shown depressive symptoms in 42.3% of the cases, with severe symptoms in 6.3%. Significantly higher total DHI and Beck scale results were observed in patients with central vestibular disorders, vestibular migraine, Persistent Postural-Perceptual Dizziness and unilateral peripheral vestibulopathy, contrasted to the results of the other four diagnosis groups. The onset of the symptoms did not significantly affect the severity of QoL worsening and depression symptoms.Conclusion:In this study, the QoL of vertiginous patients was worse in general, with the occurrence of depression symptoms. Difference was observed in case of the values of patients with different vestibular disorders, indicating the importance of different factors, e.g., central vestibular compensation, behavioural strategies and psychological factors.

This narrative review examined the literature relevant to encounters between physiotherapists and clients with suicidal thoughts and behaviours (STBs). The review was conducted in response to a growing international movement to make suicide prevention everyone's business. The Framework for Suicide Risk Assessment and Management for NSW Health Staff (New South Wales Department of Health, 2004) informed the review as it requires all New South Wales Department of Health practitioners, including physiotherapists, to be able to assess and manage clients with STBs. The review identified 23 peer-reviewed articles and four non-peer-reviewed articles that mentioned encounters between physiotherapists and clients with STBs. The results suggest that physiotherapists may encounter clients with STBs given the conditions they manage, the increased risk of suicide associated with these conditions, and the pattern of contact between health professionals and clients with STBs. Future research is required to explore the prevalence of STBs among physiotherapy clients, the experiences of physiotherapists who have identified clients with STBs, and whether physiotherapists are adequately trained and competent in suicide risk assessment.

Objective People with chronic vestibular diseases experience variable degrees of self-perceived disability. However, longitudinal data examining the predictive validity of relevant clinical variables alongside psychological variables are limited. The present study examined whether these factors predict self-reported dizziness handicap 3 months after assessment and diagnosis. Methods Patients were recruited from a waiting list of a tertiary neuro-otology clinic and completed standardized mood, cognitive, behavioral, and dizziness handicap questionnaires before and 3 months after their initial consultation and diagnosis. All patients were clinically assessed and underwent comprehensive audiovestibular investigations. Results Seventy-three percent of participants responded at follow-up ( n = 135, 73% female, mean [standard deviation] age = 54.23 [17.53] years), of whom 88% were diagnosed with a neurotological condition. There were significant improvements in handicap, depression, and anxiety at 3 months. Thirty (22%) of 135 showed clinically meaningful improvement in handicap. The percentage of case-level depression and anxiety remained the same. Negative illness perceptions and symptom responses reduced, although participants still tended to view their condition negatively. Vestibular tests and type of diagnosis were not associated with self-reported handicap. Most baseline psychological variables significantly correlated with handicap at 3 months. When adjusting for baseline handicap and demographics, the baseline psychological variables only explained a significant ~3% of the variance in dizziness handicap at follow-up, with baseline handicap explaining most of the variance. All-or-nothing behavior was the most significant predictor. Conclusions Tertiary patients with vertigo and dizziness report negative illness perceptions and cognitive and behavioral responses to symptoms that are associated with self-reported handicap over time. Future studies are needed to investigate whether targeting these factors alongside traditional treatment approaches improves handicap in patients with chronic dizziness.
