Integrated Survival Estimates for Cancer Treatment Delay Among Adults With Cancer During the COVID-19 Pandemic

Abstract: IMPORTANCECancer treatment delay has been reported to variably impact cancer-specific survival and coronavirus disease 2019 (COVID-19)-specific mortality during the severe acute respiratory syndrome coronavirus 2 pandemic. During the pandemic, treatment delay is being recommended in a nonquantitative, nonobjective, and nonpersonalized manner, and this approach may be associated with suboptimal outcomes. Quantitative integration of cancer mortality estimates and data on the consequences of treatment delay is ne… Show more

“… 66 As such, phase I trials have been disproportionately held. 65 Suspension of clinical activities has affected the ability to recruit patients, comply with protocol-mandated visits, and has affected the acquisition of laboratory and radiological data that are essential for both trial endpoints and correlative studies. 54 Many institutions have halted research-only blood draws and tissue collection and are only collecting samples that directly affect primary trial endpoints as long as patient safety is not compromised in doing so.…”

“…This would mean that patients with a new cancer are seen later, and can therefore potentially suffer from a delay in the start of their treatment. Therefore, it is important to assess the importance of such drop, as treatment delays can dramatically impact the prognosis of patients with cancer ( 7 ) ( 8 ).…”

“…Therefore, delay and/or reduction of the number of hospital and healthcare centers access is highly recommended, and it is a widely adopted strategy to reorganize therapeutic and outpatient activities of Radiation Oncology Departments during the crisis [ 70 ]. Since older adults are the population at higher mortality risk from COVID-19 and higher risk of severe consequences from COVID-19, they derive fewer benefits, in absolute terms, from postoperative RT [ 71 ].…”
