Integra in Lower Extremity Reconstruction after Burn Injury

Abstract: The use of a bioengineered, cell-free dermal matrix expands reconstructive options and allows salvage of extremities that might otherwise have been amputated or required prolonged, staged procedures. The authors' experience shows that Integra provides a sound alternative option for providing durable coverage of vital lower extremity structures. It should be considered as part of the lower extremity wound coverage algorithm.

“…Lee et al described nine cases where dermal regeneration template was used for postburn lower extremity wounds with exposed bone and tendon and hypothesized that because the dermal regeneration template is not alive when attached, a delayed revascularization can ultimately result in a graftable wound. 51 Barnett and Shilt described the use of dermal regeneration template in open tibial fractures. 52 Besner and Klamar described its use in treating purpura fulminans.…”

“…Its clinical applications have broadened to include coverage of extremity wounds with exposed tendons. [22][23][24] A recent case series of wounds with exposed tendons subsequently covered with Integra demonstrated ultimate skin graft take of 93 percent, with active range of motion of 90 percent of the contralateral side. 25 Integra prevents dessication and provides a scaffold for cellular ingrowth.…”
