Insulinoma-associated protein 1 (INSM1) is a useful marker for pancreatic neuroendocrine tumor

Abstract: Insulinoma-associated protein 1 (INSM1) is an important biomarker of Achaete-scute homolog-like 1-driven pathways. For diagnosis of pancreatic neuroendocrine tumors (PanNET), chromogranin A (CGA), synaptophysin (SYP), and neural cell adhesion molecule (NCAM) were also considered as potential biomarkers. However, it is often difficult to diagnose it immunohistochemically. Hence, we examined the expression pattern of INSM1 in pancreatic solid tumors. We detected INSM1, CGA, SYP, and NCAM immunohistochemically, i… Show more

“…To further evaluate the specificity of SYN, CgA, and INSM1, we analysed their expression in primary pancreatic acinar cell carcinoma and solid pseudopapillary neoplasm, which can histologically mimic GEP‐NEN. A prior study evaluating INSM1 expression in seven and four cases of acinar cell carcinoma and solid pseudopapillary neoplasm, respectively, had revealed none of those cases to be positive . In our cohort, INSM1 was expressed in one of the nine (11%) cases of solid pseudopapillary neoplasm, with 90% of the tumour cells being reactive.…”

“…For NET versus NEC, chromogranin A stains NEC less uniformly and less frequently compared with NET. Insulinoma‐associated protein 1 (INSM1) is a specific nuclear marker for neuroendocrine differentiation, but it does not distinguish well versus poor differentiation . Ki‐67 proliferation should not be relied upon exclusively to discriminate NET from NEC.…”

“…Patterns of immunohistochemical staining can aid in the diagnosis of neuroendocrine neoplasms and the distinction between NET, NEC, and other neoplasms with neuroendocrine differentiation. Synaptophysin and chromogranin A typically are strong and diffuse in extra‐adrenal paraganglioma and NET, even in grade 3 NET . Pan‐cytokeratin labeling is helpful for the differential diagnosis of NET and extra‐adrenal paraganglioma, which is negative.…”

“…Increased gene expression was initially demonstrated in SCLC and later in other NE neoplasms, including pheochromocytomas, medullary thyroid carcinomas, medulloblastomas, and pituitary adenomas . INSM1 protein has been found to be expressed consistently in NE and neuroepithelial neoplasms, irrespective of tumor grade, including small cell carcinomas of various sites, and has been proposed as a single marker with high specificity and sensitivity for the diagnosis of SCLC …”
