Background Chondral lesions, peroneal tendon tears, and other disorders in patients with chronic ankle instability may not be detected by preoperative MRI. Also, MRI often is obtained and interpreted at the referring institution, leading to variability in reading. Questions/PurposesWe assessed the accuracy of the radiologists' and orthopaedic surgeon's reading of preoperative MRI for diagnosing ankle lesions in patients with ankle instability warranting surgery. Patients and Methods We retrospectively reviewed 133 patients who underwent 135 surgeries for lateral ankle ligament reconstruction with concomitant ankle arthroscopy and who had preoperative MRI. Results We found 72 associated lesions in 66 of the 135 surgeries, including 38 chondral injuries, 18 peroneus brevis tears, seven loose bodies, and nine other miscellaneous abnormalities. Eliminating eight lesions for which the decision to operate was not based on operative findings, there were 127 surgeries with 64 associated lesions that required intraoperative confirmation or were detected intraoperatively. In the original reports, the radiologists identified 39% (15) of the chondral injuries, 56% (10) of the peroneal tears, and 57% (four) of the loose bodies. Radiologists' MRI sensitivity for detecting lesions was 45%. The attending surgeon identified 47% (18) of the chondral injuries, 89% (16) of the loose bodies, 71% (five) of the peroneus brevis tears, and the posterior talus process lesion, with a sensitivity of 63%. ConclusionsOur data suggest orthopaedic surgeons should review preoperative MRIs and also suggest the sensitivity of MRI may not be adequate to detect lesions in these patients before surgery.

RESuMoIntrodução: Nos esportes onde há contato entre os atletas, como no voleibol, a entorse por mecanismo de inversão é a lesão mais frequente e o tornozelo é a articulação mais acometida quando comparada com as outras, acarretando déficits importantes como a instabilidade articular. Programas de treinamento proprioceptivo têm sido amplamente utilizados para minimizar essas questões. Objetivo: Avaliar o efeito de um treinamento proprioceptivo na estabilidade articular do tornozelo por meio do teste Star Excusion Balance Test (SEBT) em atletas de voleibol. Métodos: Foram avaliadas 11 atletas de voleibol de um clube poliesportivo da cidade de Porto Alegre, RS, Brasil, por meio de um teste de avaliação para estabilidade articular do tornozelo, SEBT. As atletas passaram por um programa de treinamento proprioceptivo composto por seis exercícios que trabalharam propriocepção em diferentes graus de exigência durante quatro semanas e este teste foi aplicado no pré e pós-intervenção pelo programa de treinamento proprioceptivo das atletas. Resultados: As variáveis quantitativas foram descritas por média e desvio padrão, e o estudo da normalidade das variáveis foi descrito pelo teste de Shapiro-Wilk. As variáveis categóricas foram descritas por frequências absolutas e relativas. Para a comparação de médias de ambos os tornozelos pré e pós-intervenção foi utilizado o teste T pareado. O nível de significância adotado foi de 5% (p≤0,05) e as análises foram realizadas no programa SPSS (Statistical Package for the Social Sciences) versão 18. Os resultados no teste SEBT das atletas pré e pós-intervenção proprioceptivo mostrou diferença significativa em seis direções para o tornozelo direito e cinco para o tornozelo esquerdo. Conclusão: Um programa de treinamento proprioceptivo parece ser eficaz para o incremento da estabilidade articular que é fundamental para atletas de voleibol expostas ao risco de entorses de tornozelo que geram instabilidades crônicas. The soccer is one of the most popular sport modality around the world, with millions of practitioners of different levels, and thus, the incidence of injuries are expressive [1][2][3] . Among these injuries, the ankle sprain is the one that stands out 2, 4-5 .e ankle sprains are very common in soccer due to the demand of changes in direction performed with high velocities, after jumps and during the sprints itself 2,4,6 . ese rapid changes in direction or landing on irregular surfaces produces a large supination torque, causing the ankle to perform an excessive movement, overloading the joint and its structures, mainly the anterior talo-bular and the calcaneo-bular ligaments 7 .Repeated episodes of ankle sprains may negatively a ect the proprioception, which correspond to the perception of position and movement (i.e., synesthesia) of the body and its segments [8][9] . ese information are provided by mechanoreceptors in the muscles, tendons, skin, joint capsules and ligaments [10][11][12] .ese mechanoreceptors are sensitized by mechanics energy imposed to the joint and it is transmitted to the central nervous system by a erents impulses 13 . As a consequence to sprains, the a erent bers of the AbstractThe aim of the study was to compare the behavior of COP and passive ankle position sense in subjects with and without functional ankle instability. Took part in this study 20 subjects, divided into two groups: stable group (SG) and unstable group (UG). The COP evaluation was made with the single-leg balance test, with eyes opened and closed, on a force plate. The passive ankle position sense test was performed with subjects blindfolded. The ankle was positioned in a target angle (10° and 20°) and the dynamometer moved passively the ankle, then the subjects were instructed to push the stop button when they feel that the ankle was on the target angle, obtaining the absolute angular error (AAE). The following variables were obtained: total displacement (TD); antero-posterior (SDap) and medio-lateral standard deviation (SDml); total mean velocity (TMV); antero-posterior (MVap) and medio-lateral mean velocity (MVml). The comparison between the data with normal distribution was made with the Student's t test, while to the TD and SDml was used the Mann-Whitney test. The correlations were performed with the Pearson and Spearman tests. We adopted α < 0.05. We observed difference between AAE-10º (p < 0.05). Strong correlations were found between: AAE-10° and TMV (p < 0.01 r = -0.867); AAE-10° and MVap (p < 0.01 r = -0.854); AAE-10° and MVml (p < 0.01 r = -0.771), with eyes opened, and AAE-10° and TD (p < 0.05 r = -0.666); AAE-10º and SDap (p < 0.05 r = -0,685) and AAE-10° and MVml (p < 0.05 r= -0.766) with eyes closed. Ankle sprains harm the joint position sense without affecting the balance.
