Insights into Microbiome and Metabolic Signatures of Children Undergoing Peanut Oral Immunotherapy

Abstract: Background: Peanut oral immunotherapy has emerged as a novel, active management approach for peanut-allergic sufferers, but limited data exist currently on the role of the microbiome in successful desensitization. Objective: We examined the oral and gut microbiome in a cohort of 17 children undergoing peanut oral immunotherapy with the aim to identify the microbiome signatures associated with successful desensitization. We also set out to characterize their fecal metabolic profiles after successful therapy. Me… Show more

“…Those who respond well to immunotherapy tend to have higher alpha diversity in their gut microbiome compared to those with a poor response ( Matson et al, 2018 ). Mechanistically, this is linked to increased production of short-chain fatty acids (SCFAs), which have immunomodulatory effects ( Blackman et al, 2022 ). While alpha diversity can provide a valuable tool in predicting response to immunotherapy and developing personalized treatment plans for cancer patients, our study is the first to demonstrate that ISV that is administered directly to a tumor in contrast to systemic therapy via intravenous or intraperitoneal route, can also generate a similar therapeutic response.…”

“…The use of probiotics as adjunctive therapy during OIT could improve tolerability and decrease GI adverse effects. 69 and is currently under testing for safety, tolerability, and immunity response in a phase I study in 20 patients aged 12-17 years. 21,73 This technology vehicles synthesized Ara h directly to dendritic cells, making them available for the formation of HLA-II-antigen complexes.…”
