Insight in Schizophrenia: Relationship to Positive, Negative and Neurocognitive Dimensions

Abstract: Impairment of insight is considered as the hallmark of schizophrenia. Substantial proportion of patients with schizophrenia has either poor or absent insight. Insight is a multidimensional and dynamic construct which appears to have intricate links with other symptom dimensions of the psychotic illness. A better appreciation of the association that insight shares with other symptom clusters in psychosis could help us in gaining knowledge about aetiology, prognosis and treatment-related facets of the disorder. … Show more

“…Lack of insight is considered a core manifestation of schizophrenia (DSM-IV-TR). Presence of insight may have important implications for outcomes and recovery in schizophrenia, as individuals with poor insight may not be adherent to medications or psychotherapy (Kay et al, 1987; Karow et al, 2008; Joseph et al, 2015). Mohanty and Kumar (2012) reported in their study that CI is an important mediator of response to cognitive behaviour therapy for psychosis, acting as a predictor of improvement in positive, negative as well as general psychopathology symptoms (Granholm et al, 2005; Perivoliotis et al, 2010; Premkumar et al, 2011).…”

“…Poor insight is seen as the most common and absolute barrier to SDM among patients with schizophrenia ( 32 , 47 ) and has been linked to a poorer perceived therapeutic alliance ( 73 ). This can be explained by the description of insight as the ability of people with schizophrenia to recognize that they have an illness and their ability to understand how their experiences relate to the illness ( 74 ).…”

“…Schizophrenia is a chronic debilitating neurodevelopmental disorder that affects approximately 1% of the population worldwide; the first symptoms emerge during adolescence and early adulthood. It is characterized by a combination of positive (auditory and visual hallucination), negative (social withdrawal, anxiety), and cognitive symptoms (impaired memory, decision-making difficulties; Joseph et al, 2015 ). Following our seminal 2002 article (Andrieux et al, 2002 ), numerous studies were performed and their findings reinforced the validity of the MAP6 KO model for the study of schizophrenia.…”
