Background To test the reliability and safety of a newly invented technique for minimally invasive percutaneous nephrolithotomy, intelligent pressure-controlled minimally invasive percutaneous nephrolithotomy (IPC-MPCNL). Methods Eighteen kidneys of nine female pigs were randomly divided into three groups. Those in Groups A and B underwent IPC-MPCNL through the new system composed of a pressure-measuring MPCNL suctioning sheath and an irrigation and suctioning platform with pressure feedback control. The infusion flow rate was 500 ml/min in Group A and 750 ml/min in Group B. Those in Group C underwent MPCNL at an infusion flow rate of 500 ml/min. The renal pelvic pressure (RPP) monitored by a ureteral catheter and that monitored by the pressure-measuring sheath in Groups A and B were compared. The RPP in Group C was monitored by a ureteral catheter. Results The RPP measured by the pressure-measuring sheath and that measured by the ureteral catheter in Group A was − 5.59 ± 1.95 mmHg and 4.46 ± 2.08 mmHg, respectively. The RPP measured by the pressure-measuring sheath and that measured by the ureteral catheter in Group B was − 4.00 ± 2.01 mmHg and 5.92 ± 2.05 mmHg, respectively. Hence, the RPPs measured by the pressure-measuring sheath in Groups A and B were consistent with those measured by the ureteral catheter. The RPP in Group C was 27.75 ± 5.98 mmHg (large fluctuations). Conclusions IPC-MPCNL can be used to accurately monitor the RPP and maintain it within a preset safe range via suction. The new technique and the new system are safe and reliable.

Background: Despite the prevalence and incidence of kidney stones progressively increasing worldwide, public awareness of this condition remains unclear. Understanding trends of awareness can assist healthcare professionals and policymakers in planning and implementing targeted health interventions. This study investigated online search interest in “kidney stone” by analyzing Google Trends, focusing on stationarity of the trends and predicting future trends. Methods: We performed time series analysis on worldwide Google monthly search data from January 2004 to November 2023. The Augmented Dickey–Fuller (ADF) test was used to assess the stationarity of the data, with a p-value below 0.05 indicating stationarity. Time series forecasting was performed using the autoregressive integrated moving average to predict future trends. Results: The highest search interest for “kidney stone” (score 100) was in August 2022, while the lowest was in December 2007 (score 36). As of November 2023, search interest remained high, at 92. The ADF test was significant (p = 0.023), confirming data stationarity. The time series forecasting projected continued high public interest, likely reflecting ongoing concern and awareness. Notably, diverse regions such as Iran, the Philippines, Ecuador, the United States, and Nepal showed significant interest, suggesting widespread awareness of nephrolithiasis. Conclusion: This study highlighted that “kidney stone” is a consistently relevant health issue globally. The increase and stationarity of search trends, the forecasted sustained interest, and diverse regional interest emphasize the need for collaborative research and educational initiatives. This study’s analysis serves as a valuable tool for shaping future healthcare policies and research directions in addressing nephrolithiasis related health challenges.

Stone-free rates have drastically improved due to technological advancements in the field of endourology. One such advancement has been the use of suction evacuation sheaths (SES) during ureteroscopy (URS). Studies about outcomes after SES URS have been limited by small sample sizes. Here, we performed an analysis of postoperative outcomes of 2284 patients who underwent SES URS for the treatment of renal stone disease. We found that 86% of patients were stone-free at one month, with 50% of patients not requiring any repeat procedures for stone clearance. 1.28% experienced a major complication (Clavien-Dindo ≥ 3); 4.6% had postoperative fever, and 1.28% experienced sepsis after surgery. Ureteral injuries were seen in 1.5% of patients. Stone burden (OR 0.45 [0.33–0.62]) and the requirement of stent placement after lithotripsy (OR 0.71 [0.54–0.95]) were associated with lower SFR, while the use of basket during URS doubled the chances of being stone-free (OR 2.17 [1.33–3.53]). Age, while statistically significant, was not clinically relevant (OR 0.97 [0.96–0.98]). Our study is limited by its retrospective nature and the absence of a comparator group. Nonetheless, this study represents the largest cohort of patients to undergo SES URS for renal stone disease, and demonstrates that SES URS is a safe and efficacious technique for stone removal, with an SFR of 86% at one-month, and a 1.28% sepsis rate after surgery. Manual stone extraction in these patients can double their chances of being stone-free, thus highlighting that the use of SES alone is insufficient during URS.
