Injecting drug users in Bangladesh: prevalence of syphilis, hepatitis, HIV and HIV subtypes

Abstract: Injecting drug users (IDU) were enrolled from two detoxification clinics and two needle/syringe exchange programmes (NEP) in central and northwest Bangladesh. Syphilis, hepatitis C and HIV rates were highest in IDU from the NEP of central Bangladesh (23, 66.5 and 1.4%, respectively), whereas current hepatitis B infection rates were highest in IDU from the NEP of northwest Bangladesh (12%). Five HIV strains were subtype C and one E/B. The 32 base pair (bp) deletion of the CCR5 gene was not detected.

“…The sero-prevalence of syphilis (according to TPHA) of 3.3% in our study is comparable to the findings of studies carried out in the USA among drug abusers in treatment facilities [7,15,16], but it is much lower than 23% among injecting drug users in Bangladesh [17]. However, only 1 in 5 of the TPHA positive patients in our study (0.6% of all screened patients) was in need of treatment, all other patients were already treated or recovered from the disease; these proportions were not reported for the other countries and may significantly differ.…”

“…Previous work in Bangladesh has identified HIV as a concern among IDUs in Bangladesh [5-7,9] and among MSM in India and Pakistan [24,25]. There has not been a clear link between the high risk groups of MSM and IDUs in Bangladesh, and this is the relationship we explore in this paper.…”

“…National surveillance data for HIV from the Ministry of Health and Government of Bangladesh have reported data from a variety of groups showing the highest levels of HIV infection were identified among IDUs [5-8]. During the first four rounds of surveillance, from 1998 to 2002, the HIV prevalence increased from nearly zero to 4%.…”

“…HIV prevalence is generally low in the neighboring country of Bangladesh but is more than 5% in PWID in Dhaka, the capital of Bangladesh (Bontell et al, 2013). The epidemic in general and among PWID is subtype C-driven, and strains cluster with viruses from India and Myanmar (Azim et al, 2002; Bontell et al, 2013; Sarker et al, 2008). …”
