Injectable Platelet-Rich Fibrin as a Drug Carrier Increases the Antibacterial Susceptibility of Antibiotic—Clindamycin Phosphate

Abstract: The aim of this study was to investigate the change in clindamycin phosphate antibacterial properties against Gram-positive bacteria using the platelet-rich fibrin as a carrier matrix, and evaluate the changes in the antibiotic within the matrix. The antibacterial properties of CLP and its combination with PRF were tested in a microdilution test against reference cultures and clinical isolates of Staphylococcus aureus (S. aureus) or Staphylococcus epidermidis (S. epidermidis). Fourier-transform infrared spectr… Show more

“…Through the reasonable combination of these platelet derivatives, a synergistic potential can be obtained, or, in some cases, certain side effects can be reduced. Moreover, the fibrin matrix can serve as a carrier for certain substances [ 15 , 29 ]. Consequently, the combination of these concentrates with different substances could be useful in a clinical approach.…”

“…Previous studies have emphasized the possible use of platelet-rich fibrin as a carrier matrix. Egle et al (2022) [ 29 ] have used this product together with clindamycin phosphate (a prodrug of clindamycin without antibacterial activity), and the results demonstrated antimicrobial efficacy against Staphylococcus aureus and Staphylococcus epidermidis [ 29 ]. Thus, we considered it appropriate to combine a platelet concentrate, namely iPRF, with a multifunctional iron glycoprotein, lactoferrin, to obtain an increased antimicrobial potential.…”

“…Compared with pure CLP solution, CLP combined with PRF showed higher antibacterial activity against S. aureus and S. epidermidis with significantly lower minimum bactericidal concentrations (from 1,000 μg/mL to 62 μg/mL). Such a modified PRF-CLP scaffold could provide local antimicrobial activity within the first few hours, reducing the risk of postoperative infection ( Egle et al, 2022 ). Ercan E et al used T-PRF and collagen loaded with doxorubicin (Doxy), commonly used in periodontitis treatment, and its anti-collagenase significantly inhibits tissue destruction.…”

“…The carrier was observed to induce structural changes in the clindamycin, giving rise to a more active compound that significantly decreased the minimal bactericidal concentrations required to eliminate Staphylococcal strains. The researchers attested to the safety of the engineered carrier for human cells in vitro, thus evidencing the system’s potential to reduce the risk of postoperative infection [ 17 ]. Finally, Lee et al proposed the controlled storage and release of nitric oxide from metal organic nanosized frameworks formed from Cu-BTC for prospective uses in drug delivery systems.…”
