Initial Psychometric Testing of the Knowledge About Atrial Fibrillation and Self-Management Survey

Abstract: Background Guidelines endorse educating patients to self-manage atrial fibrillation (AF) to mitigate AF-related adverse events contributing to personal and societal burden. Published interventions to improve patients' knowledge about AF and self-management are emerging, but evaluations of interventions are limited by lack of a psychometrically sound instrument to measure learning outcomes. Objective We report results of initial psychometric testing of t… Show more

“…Patients did not encounter any particular difficulties in understanding the items of the AFKS-C. None of the items had to be modified to fit the Chinese cultural context, except for the item related to the function of the thrombosis center, which was modified to "warfarin clinic" to fit the terminology used in Hong Kong. In terms of reliability, in this study, we recorded a Kuder-Richardson formula 20 value of 0.60 for the AFKS-C, which indicated only borderline internal consistency. Other AF knowledge tests, such as the Knowledge about AF and Self-Management survey, 36 the Knowledge of AF test, 37 and the Knowledge of Atrial Fibrillation and Stroke Prevention Questionnaire, 38 have reported Cronbach α values greater than 0.80. Such differences in the internal consistency values between these tests and the AFKS-C could be due to the number of items in these tests.…”

“…16 However, the authors also did not perform adequate psychometric testing other than content and face validity. Recently, McCabe et al published the newly developed self-management instrument in patients with AF; 40 it has shown adequate content and construct validity as well as internal consistency reliability. Thus, we recommend confirming convergent validity using AF-SCS-10 and this newly developed instrument in future studies.…”

“…Knowledge scores are calculated as a percentage of correct answers, with higher numbers indicating higher knowledge. Four items were removed from the overall knowledge percent scores, as per McCabe et al [ 25 ] finding that these items had factor loadings below .45 and were not reliable predictors of overall test performance and knowledge. The Knowledge about AF tool demonstrated an internal consistency reliability coefficient of .86, convergent validity (i.e., was positively related to another knowledge about AF test), and distinguished between patients recently diagnosed with AF from those seeking advanced treatment for AF [ 25 ].…”
