Initial orthostatic hypotension and orthostatic intolerance symptom prevalence in older adults: A systematic review

Abstract: Background Initial orthostatic hypotension is a clinically relevant syndrome in older adults which has been associated with symptoms of orthostatic intolerance. The aim of this systematic review was to determine the prevalence of orthostatic intolerance symptoms in older adults with initial orthostatic hypotension. Methods MEDLINE (from 1946), EMBASE (from 1974) and Cochrane were searched to December 6th , 2019 using the terms “initial orthost… Show more

“…Our study results show that OH is common in middle‐aged and elderly patients and is present in up to 25.2% of our studied patients, among whom 45.4% had initial OH and 54.6% had sustained OH. According to previous studies, OH is seen in 19%–68% of older inpatients, 11 and the prevalence of initial OH ranges from 3.5% to 62.5% in geriatric patients aged ≥ 65 years, 12 depending on the population characteristics, comorbidities, and medications.…”

“…Initial orthostatic hypotension (IOH) is a clinical syndrome defined as a transient decrease of ≥40 mm Hg systolic blood pressure (SBP) and/or ≥20 mm Hg diastolic blood pressure (DBP) within 15 s of active standing [1]. The prevalence of IOH is 27.8% in the older general populations and 35.2% in geriatric outpatients [2] with a high 952 DOI: 10.1159/000521411 prevalence of orthostatic intolerance symptoms in older adults, with IOH ranging between 24 and 100% [3]. Unlike classical orthostatic hypotension (COH) which can be detected using a conventional sphygmomanometer [4], the diagnosis of IOH requires non-invasive, beat-tobeat, continuous blood pressure monitoring to evaluate blood pressure changes occurring within 60 s after active standing [4] or passive tilting [1].…”
