Initial Emergency Department Trauma Scores from the OPALS Study: The Case for the Motor Score in Blunt Trauma

Abstract: The initial emergency department motor score showed the highest predictive validity among all of the other components. These results suggest its validity for blunt trauma triage when compared with the GCS or RTS.

“…They assessed three methods of dealing with the verbal scoring of an aphasic patient: (1) eliminating the verbal component, (2) pseudoscoring with '1', and (3) median value substitution of the other components. Their data agrees with others' data [48][49][50][51][52][53][54], that the motor and eye components alone can accurately substitute for a complete GCS where the eye and motor subscale had 87% accuracy compared to 88% for the model with eye, motor, and verbal scale.…”

“…Previous work has been reported using linear regression analysis to predict GCS verbal scores from motor and eye subscores [51,52]. These studies have proved the extrapolation of the verbal subscore from the eye and motor components to be applicable to alterations of consciousness due to traumatic and non-traumatic etiologies [53].…”

“…Zu diesem Zwecke darf der initiale GCS am Unfallort als maßgebend erachtet werden. Die Autoren fanden, dass für den initialen Triage die motorische Komponente alleine von gleichwertiger Aussagekraft ist, wie der gesamte GCS [3]. Dies erleichtert und beschleunigt das Triage-Verfahren und löst das Problem der oft nicht eindeutig testbaren verbalen und okulären Reaktion.…”

It has been used in several studies and is reliable in determining patient`s status (Al-Salamah et al, 2004). However, there are different studies available regarding triage and categorizing patients and every study adopts a different approach for this purpose.…”
