Purpose: The mechanisms involved in the reported beneficial effects of Calcium dobesilate monohydrate (CaD) for the treatment of diabetic retinopathy (DR) remain to be elucidated. The main aim of the present study is to examine whether CaD prevents early events in the pathogenesis of DR such as neurodegeneration and vascular leakage. In addition, putative mediators of both neurodegeneration (glutamate/GLAST, ET-1/ETB receptor) and early microvascular impairment (ET-1/ETA receptor, oxidative stress, VEGF, and the PKC-delta-p38 MAPK pathway) have been examined. Methods: Diabetic (db/db) mice were randomly assigned to daily oral treatment with CaD (200 mg/Kg/ day) (n = 12) or vehicle (n = 12) for 14 days. In addition, 12 non-diabetic (db/+) mice matched by age were used as the control group. Functional abnormalities were assessed by electroretinography. Neurodegeneration and microvascular abnormalities were evaluated by immunohistochemistry and Western blot. Glutamate was determined by HPLC. Results: CaD significantly decreased glial activation and apoptosis and produced a significant improvement in the electroretinogram parameters. Mechanistically, CaD prevented the diabetes-induced upregulation of ET-1 and its cognate receptors (ETA-R and ETB-R), which are involved in microvascular impairment and neurodegeneration, respectively. In addition, treatment with CaD downregulated GLAST, the main glutamate transporter, and accordingly prevented the increase in glutamate. Finally, CaD prevented oxidative stress, and the upregulation of VEGF and PKC delta-p38 MAPK pathway induced by diabetes, thus resulting in a significant reduction in vascular leakage.Conclusions: Our findings demonstrate for the first time that CaD exerts neuroprotection in an experimental model of DR. In addition, we provide first evidence that CaD prevents the overexpression of ET-1 and its receptors in the diabetic retina. These beneficial effects on the neurovascular unit could pave the way for clinical trials addressed to confirm the effectiveness of CaD in very early stages of DR. ARTICLE HISTORY

Abstract:The rapid escalation in the global prevalence diabetes, with more than 30% being afflicted with diabetic retinopathy (DR), means it is likely that associated vision-threatening conditions will also rise substantially. This means that new therapeutic approaches need to be found that go beyond the current standards of diabetic care, and which are effective in the early stages of the disease. In recent decades several new pharmacological agents have been investigated for their effectiveness in preventing the appearance and progression of DR or in reversing DR; some with limited success while others appear promising. This up-to-date critical review of non-traditional systemic treatments for DR is based on the published evidence in MEDLINE spanning 1980-December 2014. It discusses a number of therapeutic options, paying particular attention to the mechanisms of action and the clinical evidence for the use of renin-angiotensin system blockade, fenofibrate and calcium dobesilate monohydrate in DR.

Боткина», Мамоновский переулок, 7, Москва, 123001, российская Федерация Цель -усовершенствование подходов к патогенетическому лечению диабетической ретинопатии (Др) на основе ранней диагностики и нового способа прогнозирования прогрессирования заболевания. Материал и методы. обследовано 330 пациентов (660 глаз) с сахарным диабетом (СД) 2-го типа с Др: женщин 64,6%, мужчин 35,4%, средний возраст 62,3±2,3 года. Пациенты были разделены на 3 группы: контрольная группа -здоровые лица (30 человек, 60 глаз) и пациенты с СД 2-го типа без Др (Др0) (30 человек, 60 глаз); 1-я исследуемая группа -пациенты с СД 2-го типа с ДрI без диабетического макулярного отека (ДМо) (ДрI без ДМо) (30 человек, 60 глаз), получавшие консервативное лечение ангиопротектором кальция добезилатом, зарегистрированным в россии как Докси-хем (ру № П N126627/01 от 28.03.2012); 2-я исследуемая группа -пациенты с СД 2-го типа с Др разных стадий (ДрI, II, III) с ДМо, которым выполняли лазерную коагуляцию сетчатки (лкС) (240 человек, 480 глаз). Все группы сопоставимы по полу и возрасту. Всем пациентам проведено офтaльмологическое обследование, включавшее определение максимально корригированной остроты зрения (МкоЗ), критической частоты слияния мельканий (кЧСМ), тонометрию, биомикроскопию, фундусмикропериметрию MAIA, оптическую когерентную томографию (окТ), флюоресцентную ангиографию (ФАГ) сетчатки. Традиционно определяли уровень глюкозы крови, гликированного гемоглобина в плазме крови; в слезной жидкости (Сж) анализировали уровень фактора роста эндотелия сосудов (VEGF-А) и моноцитарного хемоаттрактантного белка (MCP-1) методом твердофазного иммуноферментного анализа. Результаты. В 1-й группе после курса консервативной терапии с использованием кальция добезилата повысилась МкоЗ, составив в среднем 0,95±0,02, увеличилась кЧСМ -42,5±0,2 Гц (p<0,05). Средний показатель центральной толщины сетчатки (цТС) достоверно уменьшился -265,1±12,1 мкм (p<0,05). Световaя чувствительность мaкулы (СМ) увеличилась, составив после лечения в среднем 24,13±12,3 дБ (p<0,05). Во 2-й группе средний показатель цТС составил 383,1±22,1 мкм, что было достоверно выше аналогичного показателя как у здоровых лиц (p<0,05), так и у больных СД 2-го типа без Др (Др0) (p<0,05). Исследование Сж через 12 мес после лечения выявило значительное снижение концентрации VEGF-A до 655,1±86,1 пг/мл и МСр-1 до 1133 пг/мл, что оказалось достоверно ниже исходных значений (p<0,05). Выводы. консервативное лечение препаратом Докси-хем ДрI без ДМо является эффективным на ранней стадии Др, позволяет улучшить и стабилизировать клинико-морфологическое состояние сетчатки уже в течение 1 мес (по данным ФАГ, окТ, фундусмикропериметрии) у пациентов с СД 2-го типа. лазерное лечение у пациентов с ДМо при ДрI, ДрII, ДрIII позволяет улучшить и стабилизировать клинико-морфологическое состояние сетч...
