“…McWhorter proposed six criteria for the diagnosis of TAH, including immediate appearance after the trauma, the previous inexistence of a hernia in this region, and evidence that the trauma was sufficient to cause a lesion of this type (12). Clain, in 1964, additionally suggests that there should be no penetration of the skin, and that the physician should have been consulted while the signs of trauma were still evident (1).…”

“…The first case, due to direct impact against the handle of a wheelbarrow, was reported by Selby in 1906 (18). The first case due to impact against the handlebars of a bicycle was reported by McWhorter in 1939 (12). In a review of the English literature, we have found 20 clear cases of handlebar hernia (understood to mean hernia due to direct impact against the handlebars of a bicycle or motorcycle), 12 of these in children.…”

“…In 1939, McWhorter [8] reported a case of TAWH and proposed a list of criteria for diagnosis. These criteria were revised and simplified by Clain [9] in 1964 and became the basis for TAWH criteria used today ( Table 1).…”

“…Various criteria have been laid down from time to time to define TAWH, but of prime importance is the presence of intact skin at the site of herniation, and the absence of any hernia at the site prior to the trauma. [4,5] TAWH usually occurs as result of a direct blow to the abdominal wall, which results in disruption of the muscle layers, but the skin remains intact since the force was not sufficient enough to penetrate the skin. A tangential shearing force associated with increase in abdominal pressure has also been hypothesized to be responsible for muscle or fascial disruption.…”
