Inguinal hernia containing a native orthotopic kidney

Abstract: We report a rare case of an inguinal hernia containing part of a native kidney and present a review of the literature with regard to urological findings in patients with inguinal hernias. This case involves an elderly man with known bilateral inguinal hernias with an incidental radiographic finding of a large right inguinal hernia containing the inferior pole of the right kidney. The patient was not symptomatic from the hernia and given his overall frailty, no surgical intervention was offered.

“…Due to the patient’s presentation of persistent pain and obstructive uropathy with acute kidney injury, a decision was made to perform a surgical intervention urgently. A previous case of inguinal hernia containing a native kidney did not require intervention due to the patient being asymptomatic and frail [ 4 ]. Another case had a patient who underwent cystoscopy with no evidence of hydronephrosis.…”

“…Inguinal hernias that involve the kidney are especially unique, and literature is limited to case reports and are often in the setting of ectopic kidneys. Upon review of the literature, there are only four documented reports of an inguinal hernia containing an orthotopic kidney [ 4–7 ]. One of these cases involved a duplicate kidney and underwent operative repair of the hernia.…”

“…According to Oruç et al, 11.2% of hernias are associated with urological tumors and 23.5% are accompanied by complications [8]. Currently, only a few cases of "extreme nephroptosis" (the displacement of the kidney into the cavity of the inguinal hernia) have been reported [2][3][4][5][6][7].…”

“…Te most severe degree of renal ptosis is characterized by the kidney displacement of over three vertebral lengths and its localization in the pelvic cavity. Te literature describes singular clinical observations of "extreme nephroptosis" when the foating kidney is located in the cavity of the inguinal-scrotal hernia [2][3][4][5][6][7]. In general, the inclusion of a parenchymal organ into an inguinal hernia is very rare.…”
