A 30-year-old Pakistani construction worker, not known to have any chronic medical illnesses, presented to the emergency room with a history of ingesting two cups of cement diluted in water, seven hours prior to the presentation, in addition to a cut on his left wrist using a sharp piece of ceramic. He was conscious, oriented, and vitally stable. Physical examination was unremarkable except for epigastric hardness and tenderness. Treatment upon admission included escitalopram 10 mg and haloperidol 5 mg. Upper GI endoscopy showed large, hard cement in the stomach and multiple pre-antral erosions. The patient was started on omeprazole 40 mg after the procedure. Exploratory laparotomy and gastrotomy were performed as well. The procedure showed a foreign body, gypsum, occupying the stomach and extending from the fundus to the pylorus. Multiple small foreign bodies were seen in the rectum. The foreign bodies were extracted completely. Before discharge, a suicide risk assessment was done using the modified SAD PERSONS scale. The patient's total score was 5, which is low risk. The patient received psychiatric care, and his postdischarge follow-up was unremarkable.

Potential health risks of contaminated media linked to bioavailability and hematotoxicity of uranium-238 ( 238 U) and thorium-232 ( 232 Th) remain uncertain. This study investigates the relative bioavailability (RBA), histopathological, and hematological effects of acute oral exposure to 238 U and 232 Th in cocontaminated concrete dust using 174 female Sprague Dawley (SD) rats. In order to create a range of 238 U and 232 Th concentrations, concrete was spiked with uranyl and thorium nitrates (~ 50, 100, and 200 mg kg −1 ). Spiked concretes were then crushed, ground, sieved (≤75 µm), and blended uniformly to create co-contaminated concrete dust. SD rats' diet pellet was amended with co-contaminated concrete dust and orally ingested over a 48-hour exposure period. The RBA values of 238 U and 232 Th in post-exposure rats' blood were determined as 22.0% ± 0.86% -30.8% ± 1.01% and 11.8% ± 0.14% -13.7% ± 0.29%, respectively. Compared to 232 Th, 238 U blood levels of SD rats fed with co-contaminated concrete dustamended diets were ~ 100-fold higher due to solubility differences, and 238 U-RBA values were approximately two-fold greater, revealing that their absorption rates in the gastrointestinal tract were affected by compound solubility. Post-acute 238 U and 232 Th ingestion from co-contaminated concrete dust demonstrate noticeable histopathological and hematological alterations, implying that intake of 238 U and 232 Th in co-contaminated concrete dust can lead to erythrocytes damage and elevated hematological attributes. Our study would be bene cial for an adequate understanding of the health implications caused by the acute oral exposures of 238 U and 232 Th in co-contaminated concrete dust, especially in the bioavailability and toxicity assessment.

Endoscopic management of massive mercury ingestion

Rationale:Ingestion of a massive amount of metallic mercury was thought to be harmless until the last century. After that, in a number of cases, mercury ingestion has been associated with appendicitis, impaired liver function, memory deficits, aspiration leading to pneumonitis and acute renal failure. Treatment includes gastric lavage, giving laxatives and chelating agents, but rapid removal of metallic mercury with gastroscopy has not been used.Patient concerns:An 18-year-old man was admitted to our emergency department after drinking 1000 g of metallic mercury as a suicide attempt.Diagnosis:Except from mild umbilical tenderness, he had no other symptoms. Radiography showed a metallic density in the area of the stomach.Intervention:Gastroscopy was performed to remove the mercury. One large pool and several small droplets of mercury were removed from the stomach.Outcomes:Blood and urine mercury levels of the patient remained low during hospitalization. No symptoms of mercury intoxication developed during the follow-up period.Lessons:Massive mercury ingestion may cause several symptoms, which can be prevented with prompt treatment. We used endoscopy to remove the mercury, which shortened the exposure time and minimized the risk of aspiration. This is the first case where endoscopy was used for the management of mercury ingestion.
