The follicular thyroid carcinoma (FTC) and follicular thyroid adenoma (FTA) are malignant and benign thyroid neoplasms, respectively. MicroRNA (miRNA) expressions have been touted as an indicator for prognostic outcome in thyroid cancer. The study objective was to explore genes suppressed by miRNA-21-3p and miRNA-21-5p for potential therapeutic insights. Differentially expressed genes and their functional enrichment were obtained from 25 FTA and 27 FTC gene microarray dataset GSE82208 using R and Bioconductor tools. The miRNA target sites were obtained from miR-TarBase database. A unique gene list of differentially expressed FTC and FTA were entered into miR-TarBase database to obtain target genes for both miRNA-21-3p and miRNA-21-5p. The result showed that miRNA-21-3p and miRNA-21-5p downregulated TIMP3, MAT2A, TGFBR2, and PLAT gene in FTC and FTA leading to significant expression of acute phase-response to metallothionein, metal ions, and unfolded protein response (UPR). The computational analysis suggests that the suppression of miRNA-21-3p and miRNA-21-5p could be an intervention strategy for therapeutically targeting FTC and FTA treatments.

Wprowadzenie i cel pracy. Konsekwencją zwiększonej dostępności oraz częstotliwości wykonywania badań obrazowych stanowi coraz powszechniejsza diagnoza guzków tarczycy. Ogniskowe zaburzenia echostruktury w badaniu ultrasonograficznym mogą być widoczne u ponad połowy badanych pacjentów. Celem pracy jest próba przedstawienia najistotniejszych informacji na temat guzków tarczycy w sposób przydatny dla codziennej praktyki klinicznej lekarza, niezależnie od specjalizacji. Skrócony opis stanu wiedzy. Podstawą algorytmu diagnostycznego przy podejrzeniu obecności zmian ogniskowych tarczycy jest dokładne badanie podmiotowe i przedmiotowe pacjenta. Należy ocenić ryzyko choroby nowotworowej gruczołu tarczowego, biorąc pod uwagę narażenie na czynniki zewnętrzne oraz wywiad rodzinny. Szczególną czujność onkologiczną powinny wzbudzić zaburzenia symetrii, rozmiaru i konsystencji gruczołu oraz obecność twardych, niebolesnych i nieprzesuwalnych okolicznych węzłów chłonnych. Guzki tarczycy mogą manifestować swoją obecność, dając objawy miejscowe, związane głównie z dolegliwościami uciskowymi, oraz objawy ogólnoustrojowe, wynikające z zaburzonej produkcji hormonów. Objawy uciskowe wiążą się z ograniczeniem światła przełyku, dusznością oraz chrypką. Badanie USG, TSH oraz BACC tarczycy stanowią podstawowe narzędzia warunkujące odpowiednie postępowanie terapeutyczne, które może opierać się na prostej obserwacji po resekcję chirurgiczną. Podsumowanie. Kluczową rolą lekarza jest wyodrębnienie pacjentów z istotnym prawdopodobieństwem procesu złośliwego. Obecnie stosowane metody diagnostyczne guzków tarczycy w większości przypadków umożliwiają trafną diagnozę, pozwalając oszczędzić zbędnych interwencji chirurgicznych pacjentom z rozpoznaniem zmian łagodnych.

The relevant role of long non-coding RNAs (lncRNAs) in cancer is currently a matter of increasing interest. Medullary thyroid cancer (MTC) is a rare neuroendocrine tumor (2–5% of all thyroid cancer) derived from the parafollicular C-cells which secrete calcitonin. About 75% of all medullary thyroid cancers are believed to be sporadic medullary thyroid cancer (sMTC), whereas the remaining 25% correspond to inherited cancer syndromes known as Multiple Endocrine Neoplasia type 2 (MEN2). MEN2 syndrome, with autosomal dominant inheritance is caused by germline gain of function mutations in RET proto-oncogene. To date no lncRNA has been associated to MEN2 syndrome and only two articles have been published relating long non-coding RNA (lncRNA) to MTC: the first one linked MALAT1 with sMTC and, in the other, our group determined some new lncRNAs in a small group of sMTC cases in fresh tissue (RMST, FTX, IPW, PRNCR1, ADAMTS9-AS2 and RMRP). The aim of the current study is to validate such novel lncRNAs previously described by our group by using a larger cohort of patients, in order to discern their potential role in the disease. Here we have tested three up-regulated (RMST, FTX, IPW) and one down-regulated (RMRP) lncRNAs in our samples (formalin fixed paraffin embedded tissues from twenty-one MEN2 and ten sMTC patients) by RT-qPCR analysis. The preliminary results reinforce the potential role of RMST, FTX, IPW and RMRP in the pathogenesis of MTC.
