Influence of Treatment Technique on Dose-Volume Histogram and Normal Tissue Complication Probability for Small Bowel and Bladder A Prospective Study Using a 3-D Planning System and a Radiobiological Model in Patients Receiving Postoperative Pelvic Irradiation

Abstract: Using multiple field techniques both the dose to the organs of risk and the fractional part of risk organs within the high-dose region can be reduced significantly. As a consequence of this a lower NTCP was calculated for the 3-FT and the 4-FT than for the OFT. Using the biological model a small, but significant difference between a 3-FT and a 4-FT was demonstrated in favor to the 4-FT.

“…In this situation an optimized irradiation technique should be used to minimize the amount of small bowel in the high-dose field areas. Studies using the prone position with the BBD have reported a clear advantage in terms of small bowel volume irradiated [2,7,10]. However, most studies included patients treated postoperatively, and data concerning the potential gain in the setting of preoperative radiotherapy are scarce.…”

“…Several reports have described devices and techniques to shift small bowel from the high dose volume [3]. Recently, the use of the prone position with the belly board device (BBD), causing the small bowel to be shifted anteriorly, has gained popularity, particularly in the setting of postoperative radiotherapy for pelvic malignancies [2,7,10]. Moreover, the technique has been described as comfortable and highly reproducible [12].…”

“…In cervical cancer patients, the presence of metastatic spread to the PLNs correlates with poor prognosis. Therefore, prophylactic irradiation of the PLNs should be recommended [11,18,22].…”
