Several studies have reported that not only patients with chronic diseases but also their partners are likely to face major psychosocial problems. This study examined the association between a partner's ongoing treatment for chronic disease and the risk of psychological distress after the Great East Japan Earthquake (GEJE). In 2012, a questionnaire was distributed as part of a cross-sectional study of participants aged 20 years or older living in a municipality that had been severely inundated by the tsunami following the GEJE. We identified couples using the household numbers of the municipality and collected self-reported information on ongoing chronic disease treatment for stroke, cancer, myocardial infarction, and angina. Psychological distress was evaluated using the Kessler 6 scale (K6) and was defined as a score ≥ 5/24 points. Among 1,246 couples (2,492 participants) thus identified, 2,369 completed the K6. The number of participants whose partners were under treatment for chronic diseases was 209 (9%). Overall, participants with partners who were receiving treatment for chronic diseases (odds ratio [OR] = 1.3, 95% confidence interval [CI] = 0.95-1.8, P = 0.09) did not show a significantly higher risk of psychological distress using logistic regression analysis. Women, but not men, whose partners were receiving treatment for chronic diseases, had a higher risk of psychological distress (women: OR = 1.6, P = 0.02; men: OR = 1.0, P = 0.92). After the GEJE, only in women the presence of partners under treatment for chronic diseases appears to be a risk factor for psychological distress.

Background: Several studies have reported on increases in the incidence of cardiovascular and cerebrovascular diseases after huge earthquakes. An increase in the incidence of cerebrovascular diseases was observed after the Great East Japan Earthquake and Tsunami of 2011. To assess whether tsunami damage or the earthquake was responsible for this trend, we assessed the relative impact of earthquake magnitude and flood damage on cerebrovascular disease. Methods: A total of 12 coastal municipalities facing the epicenter were divided into 4 flood severity groups according to the percentage of people living in the flooded areas (<20, 20-40, 40-60, and ≥60%) and 3 groups according to the Japanese Meteorological Agency seismic intensity of the main shock (<4.5, 4.5-5.0 and ≥5.0). The standard incidence ratios (SIRs) of cerebrovascular diseases in the first 4 weeks after the disaster compared with the same periods in 2008−2010 were calculated for each flood severity group and each earthquake severity group. Odds ratios (ORs) of disease incidence and the adjusted ORs for seismic intensity (using the Mantel-Haenszel method) between the higher (≥40%) and the lower flooded area (<40%) were compared with the same periods in 2008−2010. Likewise, ORs and adjusted ORs for flood severity in the high seismic intensity area (≥4.9) were compared with those in the low seismic intensity area (<4.9). Results: SIRs increased with the increased flood severity: 0.94 (0.59-1.30) at <20%, 1.02 (0.70-1.34) at 20-40%, 1.26 (0.66-1.86) at 40-60% and 1.98 (1.25-2.72) at ≥60%. However, SIRs did not increase with increased seismic intensity: 0.95 (0.60-1.29) at <4.5, 1.52 (1.07-1.98) at 4.5-5.0 and 1.17 (0.80-1.54) at ≥5.0. ORs and adjusted ORs for seismic intensity in the high flood area compared with the low flood area were significant: 1.68 (1.07-2.65) and 1.78 (1.08-2.96), respectively. However, ORs and adjusted ORs for flood severity in the high seismic intensity area compared with the low intensity area were not significant: 1.33 (0.82-2.17) and 1.19 (0.62-2.31), respectively. Conclusions: Cerebrovascular disease incidences after the Great East Japan Earthquake and Tsunami of 2011 increased because of tsunami damage and not because of the earthquake magnitude.

The Great East Japan Earthquake, a magnitude 9.0 quake that occurred on March 11, 2011, left more than 20,000 killed or missing and resulted in more than 400,000 people being displaced. The Fukushima Nuclear Power Plant accident released large amounts of radioactive material into the air. Among the victims of this combined disaster were many pregnant and parturient women, and this study aimed to determine post-disaster anxiety among this specific population and measures for the future. Participants were 259 women (mean age 33.02 ± 4.79 years) who gave birth around the time of the earthquake in Miyagi Prefecture, one of the disaster areas. Sixteen months after the earthquake, we administered survey questionnaires on anxiety. We transcribed questionnaire responses, coded raw data by context, and categorized these codes by commonality. After extracting subcategories of anxiety-related factors, we categorized them into more abstract concepts. Among the participants, 126 (48.6%) reported having no available professionals with whom they could consult about childrearing. Participants reported anxiety in the following 12 categories: "radiation," "child's physical and mental growth/development," "recurrence of earthquake and tsunami," "financial issues," "childrearing environment," "living environment," "maternal employment," "stigma," "familial issues," "maternal health," "childrearing," and "the future". A beneficial way to reduce maternal anxiety in the 12 areas identified would be to develop support systems that provide continuous support for children's mental health care needs, psychological guidance, community support for maternal empowerment, outreach for individual support, and professional consultation for mothers who have high anxiety about radioactivity.
