Ann R Coll Surg Engl 2008; 90: 40-44 40Percutaneous nephrolithotomy (PCNL) has become the mainstay of treatment for large renal calculi over the past 30 years since Fernstrom and Johansson first removed a renal calculus through a nephrostomy tract in 1976. 1 The practice of PCNL, having been refined over time, continues to evolve and has largely replaced open stone surgery for the treatment of complex upper tract stones unsuitable for extracorporeal shockwave lithotripsy (ESWL) or ureteroscopy. This has been aided by advances in technology and equipment resulting in stone removal with less morbidity, shorter convalescence, and reduced cost compared with open surgery. 2,3 Excellent outcomes have been reported following PCNL with stone-free rates approaching 95%. Nevertheless, most published series of PCNL are from single centres with a dedicated sub-specialist interest in stone management.These represent optimum results achievable and may not reflect outcomes of everyday practice in non-specialised units. The aim of the study was to analyse the current practice of PCNL in our region. Patients and MethodsThe region comprises 12 participating hospital trusts. Of these, two were tertiary referral centres with a recognised expertise in stone management, while the rest (10) comprised district general hospitals (secondary centres) providing a range of urological interventions. All patients undergoing a PCNL procedure in 2002 were identified using theatre records and medical records scrutinised retrospectively. In trusts performing more than 20 procedures

Objective: In this study, we compared our experience using balloon and amplatz dilatation in percutaneous nephrolithotomy (PCNL). We also evaluated peri-and postoperative variables, including success rates. Material and methods: Conclusion:The balloon or amplatz dilatation methods have similar results with regard to efficacy, speed, and safety. However, the cost of the balloon dilator is higher than that of the amplatz dilator.

ÖZET GirişÜriner sistem taşları oldukça sık rastlanılan, geçmişte önemli morbidite sebebi olan, büyük cerrahi gereksinim ve uzun iyileşme dönemine ihtiyacı olan bir sorundu. Böbrek taşlarına açık cerrahi operasyon uygulanması son 20-25 yıla kadar neredeyse tek seçenek olarak uygulanan bir yöntemdi. Perkütan nefrolitotomi (PNL) ise artık günümüz-de böbrek taşı hastalığı tedavisi seçenekleri arasında önemli paya sahiptir. Geniş serilerde taşsızlık oranı %95'ler civarında olup uygun böbrek taşı olgularında standart tedavi yöntemi olarak kabul edilmektedir.Bu çalışmamızda, kliniğimizde uyguladığımız standart PNL yöntemi ile ilgili deneyimlerimizin sonuçları sunulmaktadır. Gereç ve YöntemlerOcak 2007-Aralık 2012 tarihleri arasında böbrek taşı olan 1310 olguda 1350 renal üniteye uygulanan standart PNL yönteminin sonuçları incelendi. Tüm hastalar operasyon öncesi tam kan sayımı, serum elektrolit değerleri, böbrek ve karaciğer fonksiyon testleri yönünden, kanama ve koagulasyon parametreleri, serolojik testleri (HBV, HCV,HIV), tam idrar tekiki ve idrar kültürü ile değerlendirildi. Antikoagulan ilaç kullanan hastalar ilgili bölümlerce konsülte edilerek operasyona hazır hale getirildi. İdrar kültüründe üreme olan hastalar operasyondan önce en az bir hafta antibiyotik tedavisine alındı ve idrar kültürleri steril olduktan sonra opere edildi. Taşların alanı en büyük çap ve bunu dik kesen çap cetvel yardımıyla ölçülüp, değerlerin çarpılmasıyla mm 2 cinsinden hesaplandı. Multipl taşlarda tüm taşlar tek tek ölçülüp toplam alan elde edildi. TeknikHastalar supin pozisyonda anestezi tarafından uyutulduktan sonra, litotomi pozisyonuna alınıp, 20F sistoskop kullanılarak üretere 6F açık uçlu üreter kateteri yerleştirildi.
