Influence of stress systems and physical activity on different dimensions of fatigue in female fibromyalgia patients

Abstract: Our findings imply that both changes in hypothalamic-pituitary-adrenal axis activity and physical activity contribute to variance in fatigue in the daily lives of patients with FMS. This study helps to paint a clearer picture of the biological and behavioral underpinnings of fatigue in FMS and highlight the necessity of interdisciplinary treatment approaches targeting biological, behavioral and psychological aspects of FMS.

“…The most important finding in this study is the significant inverse correlation between the ACTH hormone level with the number of the tender points, and it can consider as a significant predictor in 31% of FM patients. Previous studies found that there is an association between serum cortisol level and the fatigue symptom as this study confirmed the significant high score of FSS is associated with high ACTH level (31). A low serum cortisol level in FM that reported by other authors and the higher plasma levels of ACTH that found in this study indicates that there is a dysregulation of the hypothalamic pituitary adrenal axis (32).…”

“…Fibromyalgia is a chronic disease of unknown etiology characterized by pain, fatigue, depression and sleep disturbances that linked to several endogenous neurotransmitters and hormones (1)(2)(3). Fibromyalgia is a member of centralized pain syndromes by which the pain perception occurred in absence of noxious stimuli (4).…”

“…Therefore, our data supports the necessity of multimodal treatments that address fatigue and psychosocial distress in addition to PP intensity in order to improve QoL in women with endometriosis, in line with previous suggestions [11,72]. Thus, besides medical therapy [47], physical and psychological interventions might be beneficial in endometriosis treatment, as evidenced for a variety of chronic illnesses in women [75][76][77][78]. More attention should be paid to non-pharmacological approaches to manage the symptom burden of this silenced female disease.…”

“…Because the experience of fibromyalgia differs among people [14] and within the same individual over time [15], adopting patient-centred approaches is recommended for this disease [10]. Thus, future research needs to test more sophisticated and dynamic models accounting for differences both between and within individuals.…”

“…We hypothesised that, in relation to fibromyalgia severity, two pathways coexist (physical and psychological) and interplay [4,10]. The physical pathway goes from active lifestyle to physical fatigue, directly [15] and through objective (observed) and subjective (self-reported) physical fitness [16]. In the psychological pathway, resilience and catastrophizing are related to psychological distress [7].…”
