Influence of short implants geometry on primary stability

Abstract: BackgroundA correct design is needed in short implants to improve primary stability (PS) in low quality bone. This study aimed to compare PS of double thread and single thread short implants.Material and MethodsThirty implants with single thread design (PHI/SHORT-I) and 30 implants with double thread design (PHIA/SHORT-I) (Radhex®, Inmet-Garnick S.A., Guadalajara, Spain) were placed in 30 randomly selected bovine ribs. PS was assessed in implant stability quotients (ISQ) and periotest values (PV) with Osstell™… Show more

“…In this context, the modifications on the implant macrostructure have been developed to facilitate the achievement of primary stability in these sites ( 7 - 9 ). Parameters such as the connection, thread, and platform design have been improved, making it possible to place implants in borderline conditions ( 5 , 8 ).…”

“…Although it may not be clinically relevant to test short implants in a large bone height, this eliminates the risk of having to perform bone augmentation procedures, with possible complications, in the control group with longer implants. One could hypothesize, with short implants it is more challenging to obtain initial stability than with longer implants, possibly resulting in early implant loss (de Oliveira et al, 2016;González-Serrano et al, 2018). However, to the authors' knowledge, there are no randomized controlled clinical trials in which short implants are compared with longer implants without the combination of an augmentation procedure in the posterior region of maxilla and mandible.…”

“…In addition, low quality bone is known to be the determinant factor for SI success, as it compromises primary stability at placement (11). The short implant models used had bicortical anchorage, which may have favored the stress values observed.…”

“…Despite the favorable results of SI, the disadvantages include a lower implant surface area, leading to a smaller bone/implant interface that increases the stress concentration on bone (9). However, currently, implant surface treatment and implant geometry modifications have improved the SI performance (10,11).…”
