Influence of pregnancy on cardiac function and hemodynamics in women with Ebstein's anomaly

Abstract: Although pregnancy was relatively safe among women with Ebstein's anomaly, some women developed cyanosis, arrhythmia, and heart failure, leading to elective cesarean section. Monitoring clinical and hemodynamic changes throughout pregnancy is advised to minimize maternal cardiac risk and select the appropriate mode of delivery.

“…As a result, pregnant patients should be closely monitored, and appropriate interventions should be carried out based on the severity of their complications. Also increased rates of adverse fetal outcomes especially low birth weight and prematurity in women with diverse types of heart disease have been observed in literatures (21). Low cardiac output and cyanosis are risk factors for adverse fetal outcomes.…”

“…The Japan Circulation Society guidelines (28) suggest that elective CS should be considered for women who have cyanosis, uncontrolled arrhythmias or unstable hemodynamics. However, although it is difficult to evaluate objectively the symptoms associated with right heart failure that characterize Ebstein's anomaly, it is important to select the appropriate mode of delivery (21). In this present study both first and second case Caesarean delivery was done due to obstetrician indication and patient's preference.…”

“…EA is cyanotic congenital heart disease of the tricuspid valve and is mainly caused by malformation of the Atrioventricular valve leading to a large atrial chamber in contrast to the ventricle of the right side. This anomaly is expected in 1 of every 200,000 births (Kanoh et al, 2018;Sharma et al, 2018;Sharma et al, 2020). Characterization of the disease is commonly recognized with clinical findings of tricuspid leaflet displacement, another additional finding such as atrial septal defect (ASD) or a patent foramen ovale (PFO), and abnormal conduction system presented with an accessory pathway (Kanoh et al, 2018;N.…”
