Background: Padma 28 is an herbal formula from Tibetan Medicine, which since 35 years has been registered in Switzerland as a drug for the symptoms of circulatory disorders. Over this time, a large body of scientific literature has accumulated. The aim of this article was to give an overview of the clinical studies. Methods: A systematic literature search was done in PubMed, EMBASE, and the Cochrane Central Register of Controlled Trials (CCRCT). The clinical trials found were assessed regarding fields of application, efficacy, and safety, as well as methodological quality and level of evidence. Results: 29 trials (1 meta-analysis, 21 controlled trials, 7 open trials) and 3 retrospective case studies were found. They deal with different indications and include a total of 1,704 verum (of these, 697 children), 333 placebo, and 394 untreated or healthy patients. Dropouts and withdrawals were 2.5 and 3.5 times higher in the placebo than in the verum group, respectively. The highest level of evidence for the use of Padma 28 was found in the indication of intermittent claudication (11 trials). Indications of efficacy were also found in other vascular (6 trials) and different inflammatory diseases (12 trials). Conclusions: The results suggest a favorable safety profile for Padma 28, also in the children examined (41% of the study population). Furthermore, the results show a broad field of applications. According to clinical evidence, Padma 28 has shown to be a safe and effective symptomatic treatment option for atherosclerosis-related diseases such as intermittent claudication. It also seems to have a potential for application in certain chronic inflammatory diseases such as recurrent respiratory tract infections, viral hepatitis, and multiple sclerosis. However, further randomized controlled trials (RCT) are needed to confirm these findings.

Introduction: Tibetan medicine (TM) is a whole systems medical approach that has had growing interest in the West. However, minimal research, particularly with cancer, has been conducted. The purpose of this article is to provide an overview of TM and describe a clinical case review study to obtain preliminary evidence of TM's safety and effect on patients treated for cancer or hematologic disorders. Methods: A retrospective case review was conducted in India and cases met the following inclusion criteria: (a) confirmed diagnosis of cancer or hematologic disorder by standard Western biomedical diagnostic tests, (b) either treated exclusively with TM or received insufficient Western treatment followed by TM and (c) were in remission or had stable disease at least 2 years after start of TM. Results: Three cases were identified, 1 solid tumor and 2 hematologic diseases: Case 1-poorly to moderately differentiated adenocarcinoma of the stomach, positive lymph nodes and mucosal infiltration, with clear scans and excellent quality of life 29 months later ; Case 2-chronic myelogenous leukemia with normalization of hematologic labs within 3 months of starting TM and stable 4 years later; and Case 3-red cell aplasia improved significantly and reversed dependence on blood transfusions with TM. None of the cases experienced demonstrable adverse effects from TM. Conclusions: This limited case review found TM to be safe and have positive effects on quality of life and disease regression and remission in patients with cancer and blood disorders. Further exploration and investigation using rigorous methods is warranted.

Diabetes mellitus Typ 2 hat aufgrund seiner hohen Prävalenz, seines chronischen Verlaufs und seiner Folgeerkrankungen eine grosse medizinische, aber auch eine sozioökonomische Bedeutung. Es handelt sich um eine komplexe Erkrankung mit einer Vielzahl an pathogenen Faktoren, wobei neben der metabolischen Störung vor allem auch chronisch entzündliche Prozesse beteiligt sind. Oxidativer Stress und die Bildung von Advanced Glycation Endproducts (AGE) spielen eine zentrale Rolle bei der Entstehung und dem Fortschreiten diabetischer Folgeerkrankungen, wie z.B. mikro- und makrovaskulärer Störungen, akzelerierter Arteriosklerose, diabetischer Neuro-, Nephro- und Retinopathie sowie anderer assoziierter chronisch entzündlicher Erkrankungen. Die pathogenen Mechanismen sind komplex; sie beinhalten redundante Signalwege sowie positive Rückkoppelungen und verstärken sich gegenseitig. Daher sind zur Behandlung und Prävention vor allem Arzneimittel geeignet, die ebenfalls ein netzwerkartiges, pleiotropes Wirkmuster haben und an mehreren Stellen im Krankheitskomplex angreifen. Phytotherapeutika besitzen in der Regel eine solche komplexe Wirksignatur. Ein Beispiel für ein solches Phytotherapeutikum mit komplexer Signatur ist das Arzneimittel Padma® 28, das in der Schweiz nach den Traditionen der Tibetischen Medizin hergestellt wird. In der vorliegenden Arbeit werden die Wirkmechanismen dieses Vielstoffgemischs in den Kontext der pathogenen Vorgänge des Diabetes mellitus und seiner Folgeerkrankungen gestellt und diskutiert. Unter diesen scheinen vor allem vielfältige direkte und indirekte antioxidative Effekte, die Hemmung der Bildung von AGE und Advanced Oxidation Protein Products (AOPP) sowie die antientzündliche Aktivität im Vordergrund zu stehen. Das Wirkprofil von Padma 28 scheint damit über arterielle Durchblutungsstörungen hinaus auch die bei Diabetes verstärkten und beschleunigten pathogenen Prozesse günstig beeinflussen zu können. Die Resultate weisen auf einen möglichen Nutzen des Präparats auch bei Diabetes-bedingten vaskulären Störungen und assoziierten chronisch entzündlichen Erkrankungen hin.
