Zusammenfassung: Onychomykosen sind weltweit verbreitet. Epidemiologische Untersuchungen beziffern den Anteil von Nagelpilzerkrankungen unter den Oberfla È chenmykosen mit bis zu 30 %. Onychomykosen werden von Patienten als sto È rend und einschra È nkend in bezug auf Alltag und Sozialkontakte angesehen. Seit Beginn der neunziger Jahre wurden moderne Behandlungsmethoden entwickelt. Neben oralen Antimykotika spielen die antimykotischen Lacke wie Ciclopirox-Lack, der in 45 La È ndern der Erde eingesetzt wird, eine groûe Rolle. Der Lack ist zur Behandlung aller Formen der Onychomykose zugelassen und wirkt fungizid und sporozid. Die Wirksamkeit als Monotherapeutikum, die sich auf alle potentiellen Auslo È ser einer Onychomykose erstreckt, wurde in zahlreichen Studien mit unterschiedlichem Patientenklientel belegt. Durch den substanzspezifischen Wirkmechanismus von Ciclopirox als Nicht-Ergosterolsynthesehemmer ergeben sich in Kombination mit oralen Antimykotika synergistische Effekte. Die Applikation auf die Na È gel erfolgt mit Hilfe eines Pinsels, was den Lack fu È r Patienten zu einem einfach anwendbaren Therapeutikum macht. Die seit der Markteinfu È hrung selten berichteten, lokal begrenzten Begleiterscheinungen lassen den Ciclopirox-Lack als nebenwirkungsarm gelten.
