Influence of Intraoperative Radiotherapy (IORT) on Perioperative Outcome after Surgical Resection of Rectal Cancer

Abstract: Intraoperative radiotherapy appears to be a safe treatment option in patients with locally advanced or recurrent rectal cancer with acceptable complication rates. The effect on local recurrence rate has to be estimated in long-term follow-up.

“…Klink et al evaluated retrospectively 162 patients with LARC of whom 52 received IORT and the remainder were treated with resection alone. The authors did not find any difference in the perioperative complication rates between the 2 groups (85). Prospective randomized trials are needed to properly evaluate the benefit and toxicity of IORT in patients with locally advanced or recurrent rectal cancer.…”

“…Reports with reirradiation with or without hyperthermia or IORT (Intraoperative Radiotherapy) and surgery reported up to 40% 5‐year survival with 5–15% >grade III toxicity and excellent palliation . There have been many reports of use of IORT as an adjunct to surgery in this situation with reasonable local control rates . In a more recent series the rate of local failure was reportedly higher at 53%.…”
