Aim: The aim of this study was to assess the effect of degree of curvature on extrusion of debris from root canals; and to compare the amount of extruded debris after instrumentation with WaveOne Gold reciprocating (WOG; Dentsply Maillefer, Ballaigues, Switzerland) and OneShape rotating (OS; MicroMega, Besançon, France) single file systems. Materials and methods: Sixty mesiobuccal canals were assigned to 2 equal groups according to the degree of curvature, whether severe (25-40°) or moderate (10-15°) as measured using Schneider method. The canals in each group were further subdivided into 2 subgroups (n=15) according to the instrument used for preparation; WaveOne Gold (WOG) or OneShape (OS) files. The extruded debris was collected in preweighed glass vials. Data were statistically analyzed using One-way ANOVA and Tukey's post-hoc tests. Results: The least amounts of debris were obtained with WaveOne Gold instruments in both curvatures, with no significant difference between them (P > 0.05). Larger amounts of debris occurred with OneShape instruments in severely curved canals; but this difference was of no statistical significance. Statistically significant differences were recorded only when OneShape instruments were used in moderately curved canals. Conclusions: Debris extrusion occurs independent of the motion or design of the instrument. However, moderate root canal curvatures allowed more extrusion of debris than severe curves. WaveOne Gold results were not affected by the degree of canal curvature; and outperformed OneShape instruments in both types of curves.

Nickel-titanium (NiTi) rotary instruments are commonly used for endodontic practice today. These instruments offer many advantages, including flexibility over conventional stainless steel files. 1 Despite their undeniably favorable qualities, there is a potential risk of unexpected fracture. Many variables might contribute to file separation, but the two main causes are cyclic fatigue and torsional stress. 2 Recent attempts have used thermal treatments to modify the phase transition temperatures, and thereby enhance the mechanical properties of the files. 3 In addition to this, file manufacturers have tried to develop new designs, manufacturing processes, and kinematics to minimize fracture occurrence. 1 Moreover, the use of reciprocating motion was shown to extend the lifespan of NiTi instruments, and thus resistance to fatigue, compared with continuous rotation. 4 Reciproc R25 (Rec) (VDW, Munich, Germany) and WaveOne Primary (WO) Sirona Maillefer, Ballaigues, Switzerland) are two well-known reciprocating single-file instruments. 5 Rec and WO have the same nominal size (tip size of 25 with a 0.08 taper). The taper is constant in the apical 3 mm of the instruments, Abstract Aim: The aim of the present study was to evaluate the resistance to cyclic fatigue of heat-treated single files used in proper or in reciprocating optimum torque reverse motions. Methods: Ninety-six HyFlex electrical discharge machining (EDM), Reciproc R25 (Rec), WaveOne Primary (WO), and WaveOne Gold (WOG) Primary were used.Resistance to cyclic fatigue was determined by recording time to fracture in a stainless steel artificial canal. Differential scanning calorimetric analysis was performed.Data were analyzed by two-way analysis of variance and Bonferroni post-hoc tests at 0.05. Results:In both movements, HyFlex EDM showed higher time to fracture than other files (P < 0.0001). Rec showed higher time to fracture than WO (P < 0.0001), while WOG showed higher cyclic fatigue resistance than Rec and WO with both movements (P < 0.005). Conclusions:Reciprocating optimum torque reverse motion improved significantly cyclic fatigue resistance of gold and controlled memory wire single files. Background:Apically extruded debris produced during root canal preparation can induce postoperative inflammation and subsequent failure of root canal treatment. Therefore, debris production must be reduced to improve the outcome of root canal treatment.Aim:This study aimed to provide a theoretical basis for the improvement of root canal treatment by comparing the difference in the amount of apically extruded debris produced during the root canal preparation of extracted human teeth with ProTaper Universal (PTU), ProTaper Next (PTN), and WaveOne (WO).Materials and Methods:On March 30, 2017, three researchers searched five electronic databases (PubMed/Medline, Cochrane, Science Direct Online, Embase, and Web of Science) with no time limitations. Only articles written in English were retrieved, and 150 articles were obtained. Then, the three researchers independently selected articles in accordance with previously established inclusion and exclusion criteria, and inconsistent results were discussed. Data were analyzed through meta-analysis for standardized mean difference (SMD).Results:Eight studies met the inclusion criteria and were subjected to qualitative analysis. Four articles showed that PTU produced higher amounts of apical debris than WO; two articles showed that PTU produced lower amounts of apical debris than WO; and three articles showed that PTN produced lower amounts of apical debris than WO. Moreover, one article demonstrated that PTU produced higher amounts of apical debris than PTN. A meta-analysis showed that PTU and WO did not produce significantly different amounts of apically extruded debris: SMD = −0.26, Z = 0.50 (P > 0.05). PTU produced lower amounts of apically extruded debris than WO: SMD = −4.98, Z = 2.79 (P < 0.05). However, results were significantly heterogeneous among all the included studies (I2= 97%). No significant difference was found between PTU and WO in the amounts of apically extruded debris: SMD = 0.47, Z = 1.06 (P > 0.05).Conclusion:Currently available evidence shows that PTN can produce lower amounts of apical debris than WO (P < 0.05). Moreover, the amounts of apically extruded debris produced by PTU and WO are not significantly different (P > 0.05). Nevertheless, given the limited number of studies reviewed, a definitive conclusion cannot be reached.
