A 34-year-old woman showed liver dysfunction for the first time at 3 months after delivery. Two years later, she was referred to our department with continued liver dysfunction. She fulfilled the criteria for primary biliary cirrhosis (PBC)-autoimmune hepatitis (AIH) overlap. Liver dysfunction improved after administration of ursodeoxycholic acid and bezafibrate. To the best of our knowledge this represents the second report of PBC-AIH overlap after delivery and we discuss immunological changes during the perinatal period.

Klinika za gastroenterologiju i hepatologiju, Klinički centar Srbije, Beograd, Srbija SažetakAutoimunski hepatitis (AIH) je imunološki posredovana progresivna nekroinflamacija jetre, koja se karakteriše hipergamaglobulinemijom, autoantitelima i histološki graničnim hepatitisom. Prvi put je opisan 1950. godine, kada je pokazano da adekvatno lečen AIH ima dobru prognozu, dok u suprotnom vodi u terminalnu insuficijenciju jetre. Bolest prolazi kroz periode remisije i egzarcebacije, a ređe se manifestuje akutnom insuficijencijom jetre. Dijagnoza se zasniva na kombinaciji biohemijskih, seroloških i histoloških parametara, kao i isključenju drugih oboljenja jetre.Standardna terapija AIH se sastoji od primene kortikosteroida i azatioprina. Pokazano je da ova terapija ima uspeha kod oko 80% pacijenata. U poslednje vreme se sve više ispituju alternativne terapijske metode kod pacijenata koji nisu reagovali na standardnu terapiju ili je došlo do razvoja neželjenih efekata. U grupi lekova tzv. "druge linije" za lečenje AIH su mikofenolat mofetil, ciklosporin, takrolimus i dr.Ključne reči: autoimunski hepatitis, imunosupresija, bolest jetre, autoantitela To je retko oboljenje koje se karakteriše hroničnom imunskom inflamacijom hepatocita. Ukoliko se ne prepozna i ne leči progredira u cirozu i terminalnu insuficijenciju jetre. Kliničko ispoljavanje AIH je varijabilno i uključuje asimptomatsko povećanje aminotransferaza i nespecifične simptome kao što je mučnina, malaksalost, artralgije, simptomi i znaci ciroze jetre i fulminantne hepatične insuficijencije. Uprkos novim saznanjima, AIH je i dalje klinički, dijagnostički i terapijski izazov, s obzirom da je etiologija i dalje nejasna [2][3][4]. EpidemiologijaTipično se AIH otkriva kod žena u drugoj ili trećoj deceniji živo-ta. Noviji epidemiološki podaci, međutim, ukazuju da je u 72% slučajeva bolest dijagnostikovana u uzrastu preko 40 godina, a prosečan uzrast kada se bolest najčešće dijagnostikuje je 50 godina.

Hepatitis autoinmune en niños. Perspectivas actualesAutoimmune hepatitis in children. Current perspectives RESUMEN La hepatitis autoinmune es una enfermedad inflamatoria crónica progresiva del hígado de etiología desconocida. Afecta predominantemente al sexo femenino en la etapa prepuberal. Se caracteriza por presentar niveles elevados de transaminasas e inmunoglobulina G, niveles bajos del factor 4a del complemento y de IgA circulantes, autoanticuerpos en suero, prevalencia del antígeno leucocitario humano B8, haplotipos DR3 y DR4, y hepatitis de interfase en la histología. El curso puede ser fluctuante, con períodos de remisión espontánea. Se describen dos tipos de acuerdo con los autoanticuerpos hallados en suero. El mecanismo de producción del daño hepático es secundario a reacciones inmunes contra antígenos hepáticos no controladas adecuadamente por las células T reguladoras. La mayoría de los pacientes responden favorablemente al tratamiento inmunosupresor. Librada a su evolución espontánea, la enfermedad progresa hacia la destrucción hepática, por lo que, en su etapa terminal, requiere un trasplante hepático.
