Background: Chairside systems are becoming more popular for fabricating full-ceramic single restorations, but there is very little knowledge about the effect of the entire workflow process on restoration fit. Therefore, this study aimed to compare the absolute marginal discrepancy (AMD) and the full internal fit (FULL) of all-ceramic crowns made by two chairside systems, Planmeca FIT and CEREC, with detailed and standard mill settings. Methods: One upper molar was prepared for an all-ceramic crown in human cadaver maxilla. Full-arch scans were made by Emerald or Omnicam four times each. Twenty-four e.max crowns were designed and milled by the Planmill 30s or 40s or CEREC MCXL mills with either detailed or standard settings. The cadaver tooth was extracted, and each crown was fixed on it and scanned by a high-resolution microCT scanner. The AMD and FULL were measured digitally in mesio-distal and bucco-lingual 2D slices. The actual and predicted times of the milling were also registered. Results: No differences were observed between detailed or standard settings in either system. The AMD was significantly higher with CEREC (132 ± 12 μm) than with either Planmill 30s (71 ± 6.9 μm) or 40s (78 ± 7.7 μm). In standard mode, the FULL was significantly higher with CEREC (224 ± 9.6 μm) than with either Planmill 30s (169 ± 8.1 μm) or 40s (178 ± 8.5 μm). There was no difference between actual and predicted time with the two Planmeca models, but with CEREC, the actual time was significantly higher than the predicted time. The 30s had significantly higher actual and predicted times compared to all other models. Across all models, the average milling time was 7.2 min less in standard mode than in detailed mode. Conclusions: All fit parameters were in an acceptable range. No differences in fit between Planmeca models suggest no effect of spindle number on accuracy. The detailed setting has no improvement in the marginal or internal fit of the restoration, yet it increases milling time.

To measure the internal fit of the computer-aided designed/computer-aided manufactured (CAD/CAM) crowns, a new scanner-based chairside approach was investigated in patients, and the results were compared to the established silicone replica technique and a digital laboratory replica method. Thirty full-coverage crown preparations were included. Based on a digital impression with an intraoral scanner (IOS, Trios 3), three CAD/CAM measurement copings (‘COM’, resin composite; ‘ZIR’, zirconium dioxide; ‘NPA’, non-precious alloy) were fabricated for each tooth preparation. The internal fit of the measurement copings was analyzed with three different evaluation methods: IOS-based digital approach (D-IOS), digital replica method with laboratory software (D-GOM), and conventional silicone replica technique (CV-SR). The congruence between the determined target parameter of the 80-µm cement space and the actual measured internal gap was investigated. Statistical analysis was performed by ANOVA (p-value < 0.05). No significant difference was determined between the three evaluation methods. However, significant differences were observed for the three coping materials (p-value < 0.05), the single measurement position (marginal, axial, and occlusal fit) (p-value < 0.05), and the interaction between the coping material and the measurement position (p-value < 0.05). COM revealed the smallest internal gap, followed by ZIR and NPA. Regardless of the coping material, the occlusal gap was higher than the axial and marginal gaps. Furthermore, only the internal gaps of the marginal area almost matched the target parameter of 80-µm for the cement space. D-IOS is effective for measuring internal fit of single crowns in different clinical settings.

Objective: The purpose of this experimental in vitro study was to assess the effect of having one or two intact axial walls on the improvement of the fracture resistance, and marginal and internal adaptation of computer-aided design/computer-aided manufacturer fabricated ceramic endocrowns.Materials and Methods: Thirty-six endodontically treated mandibular molars were divided into three groups (n = 12). Group A, represented teeth that all of their axial walls were reduced till they all had 3 mm height. In group B, the buccal wall had 5 mm height and the others were reduced to 3 mm. Specimens of the group C had 5-mm-height buccal and one 5 mm-height proximal wall, with all other walls of 3 mm height. All teeth were scanned using intraoral scanner, and endocrowns were milled from zirconia lithium silicate ceramics blocks. The marginal and internal discrepancy of restorations were evaluated with the replica technique. Fracture resistance was also measured after cementation and modes of failure were observed. One-way analysis of variance and Tukey HSD multiple comparisons were used to analyze the data (α = .5).Results: Significant differences were observed within the groups in terms of the axial discrepancy (p = .022); group A had significantly higher amounts of axial discrepancy compared to group B (p = .001) and group C (p = .003). Preservation of the intact axial walls did not result in a statistically significant decrease in marginal (p = .21) and pulpal (p = .31) discrepancy values. Also, concerning the fracture resistance no significant difference was observed among the groups (p = .51). Conclusion:Preservation of at least one of the axial walls could reduce the amount of the axial discrepancy and, therefore, improves the adaptation of the restoration.However, based on this study, it did not improve fracture resistance.
