Non-steroidal anti-inflammatory drugs occupy a leading position in the pharmaceutical market, but their class-specific side effect (ulcerogenic action) significantly limits their widespread use. The N-(4-methoxybenzyl)-4-methyl-2,2-dioxo-1H-2λ6,1-benzothiazine-3-carboxamide derivative (compound NI-9) showed a pronounced analgesic effect in various models of pain syndromes. The aim of the study was to determine the effect of a new original derivative of 4-methyl-2.2-dixo-1H-2λ6,1-benzothiazine-3-carboxylic acid on the macroscopic state and biochemical parameters of the gastric mucosa of rats. The study was performed on 51 male Wistar rats. Compound NI-9 and the reference drug meloxicam were administered intragastrically once daily for 28 days at doses of 3 and 5 mg/kg, corresponding to their ED50 analgesic activity. Macroscopic indicators of GM damage, glycosaminoglycan content, phospholipid profile, level of MDA and CGP, as well as nitrates and nitrites and H2S were determined. The results were processed in the program STATISTICA 10.0 using non-parametric methods. The results showed that the carboxamide derivative was safer for the stomach because the ulcer index of compound NI-9 was 1.73 times lower than that of meloxicam. The damaging effect of the compound was more pronounced at the pre-epithelial and epithelial levels of GM protection, while the post-epithelial level (production of vasodilating molecules NO and H2S) was practically unaffected by this derivative, unlike meloxicam, which caused damage at all levels of protection. The obtained data supplement the data on the pharmacodynamics of the 4-methyl-2.2-dixo-1H-2λ6,1-benzothiazine-3-carboxylic acid derivative and determine the expediency of its further studies as a potential non-opioid analgesic.

Ризик розвитку серцевосудинних захворювань у популяції зумовлений низкою факторів, части на з яких може бути повністю чи частково модифікованою (паління, ожиріння, гіподинамія, порушення ліпідного балансу, гіпертензія, цукровий діабет та інсулінорезистентність), а частина не підлягає корекції (генетична/сімейна схильність, вік і стать). Мета дослідження – узагальнити дані літератури про статеві відмінності епідеміології серцево судинних захворювань, оцінити частоту розвитку та клінічні особливості їхнього перебігу, дати характеристику ролі статевих гормонів у механізмах кардіоваскулярних порушень та окреслити можливі шляхи персоніфікованої профілактики та лікування. Особи чоловічої статі молодого та середнього віку більш схильні до розвитку серцевосудинної патології, ніж особи жіночої статі аналогічної вікової групи, що пов’язують з різними біологічними ефектами статевих гормонів і зміною їхньої активності з віком: естрадіол посилює утворення вазоди лататорів, зменшує активність процесів вільнорадикального окиснення, має антиапоптичний і проти запальний ефекти, тоді як тестостерон індукує вазоконстрикцію, активує процеси пероксидації ліпідів і протеїнів, має проапоптотичну дію. У чоловіків збільшення серцевосудинного ризику з віком асо ціюється з розвитком атеросклеротичного процесу, тоді як у жінок серцевосудинна система захищена протягом дітородного віку, що супряжено з геномним (контроль експресії вазоконстрикторів – ангіотензину II, ендотеліну І і катехоламінів) і негеномним (вплив на судинні, ниркові та серцеві клітини) ефектами естрогенів. Незалежними чинниками виникнення та прогресування серцево судинних захворювань є зростання в крові сірковмісних амінокислот гомоцистеїну й цистеїну та зниження вмісту гідроген сульфіду. У здорових дорослих чоловіків уміст гомоцистеїну та цистеїну в крові достовірно вищий, а вміст Н2S – нижчий, ніж у жінок. Після настання менопаузи дефіцит естрогенів призводить до експоненціального зростання серцевосудинного ризику. Таким чином, статеві гормони формують анатомофізіологічний і патобіохімічний патерн схильності та проявів серцевосудинної патології в осіб різної статі, а модуляція рівня статевих гормонів в організмі може змінювати опірність організму до патологічних процесів. Подальше вивчення молекулярних механізмів, які інтегровані до формування статевих особливостей кардіоваскулярної патології дасть можливість розробити нові підходи щодо корекції статьасоційованої патології серця та судин, а також її персоніфікованого лікування та профілактики.
