Influence of conditioning regimens and stem cell sources on donor-type chimerism early after stem cell transplantation

Abstract: We retrospectively analyzed very early chimerism before and ongoing neutrophil engraftment (days 7, 14, 21, 28) and investigated the influence of conditioning regimens and stem cell sources on donor-type chimerism in 59 Japanese patients who had received allogeneic hematopoietic stem cell transplantation. The percentage of donor-type chimerism increased before engraftment in all patients who achieved engraftment. The average percentage of donor-type chimerism in patients who had received reduced-intensity stem… Show more

“…The results reported here are similar to those reported by others. [2][3][4][5][6][7][12][13][14] These data undermine the use of the terms 'myeloablative' and 'nonmyeloablative'. As Deeg 15 has recently pointed out, all transplantations in patients with malignant hematological disorders are designed to be myeloablative in order to achieve disease eradication.…”

“…Cytogenetic abnormality is one of the most widely recognized risk factors in AML patients. AMLs with karyotype t(8;21), inv (16), t(16;16) and t (15;17) are expected to be controlled by chemotherapy alone, and patients with these AMLs are thus categorized into a favorable cytogenetic risk group [3]. For this reason, indication for allo-HSCT for patients in this group is considered not at the first complete remission (CR1) but at the state of relapse, after the second remission (CR2) or in the case of failure to achieve remission [4][5][6][7].…”

“…Thus, transplant characteristics should be considered when evaluating the results of chimerism analysis. Although numerous studies have performed chimerism analysis after allogeneic PBSCT with RIC regimens [9,10,14], only limited data are available regarding the chimerism status after allogeneic BMT with RIC regimens [13]. In Japan, peripheral blood stem cell donation from unrelated donors was not permitted until October, 2010.…”

“…The kinetics of chimerism are influenced by various transplant characteristics such as conditioning regimens and stem cell sources [7,9,12,13]. Thus, transplant characteristics should be considered when evaluating the results of chimerism analysis.…”
