Influence of Combination Antiretroviral Therapy on HIV-1 Serological Responses and Their Implications: A Systematic Review and Meta-Analysis

Abstract: We aimed to analyze HIV-1 seroreversion caused by combination antiretroviral therapy (cART) and to explore antibody levels of anti-HIV-1 as an alternative biomarker of HIV-1 reservoir. We searched PubMed, Embase, the Cochrane Library, and Web of Science up to August 2021 for publications about the performance of HIV-1 serological assays or the association between antibody responses against HIV-1 and HIV-1 reservoirs. Potential sources of heterogeneity were explored by meta-regression analysis, including the ye… Show more

“…Specifically, a fixed-effect model will be adopted if between-study heterogeneity is homogenous (ie, <25%), while a random-effect model will be applied if between-study shows evidence of heterogeneity (ie, ≥25%). Funnel plot asymmetry and Berg’s and Egger’s tests will be used to assess the risk of bias 36. Funnel plot graphs will be used to visually assess publication bias 37…”

“…Apart from advanced HIV disease [4], other reported reasons for a negative HIV antibody test in an HIVpositive individual include: non-adherence to WHO testing guidelines; retesting among known people living with HIV on antiretroviral therapy (ART); and testing during the window period or early stage of infection where the HIV antibody response has not yet developed [2,5]. Moreover, a recent meta-analysis found initiation of combined ART within the early/acute period of HIV infection in adults or before the age of 6 months in children, to be associated with a greater likelihood of a negative antibody test later on [6].…”
