Influence of cognitive reserve in schizophrenia: A systematic review

Abstract: To perform a systematic review of the influence of sociobehavioural indicators of cognitive reserve (CR) in the risk of schizophrenia, the clinical manifestations of the disease, and cognitive intervention programs (CRT) carried out with these patients. Method: A cross search was made by two independent reviewers in Pubmed and PsycINFO databases using keywords "schizophrenia" and "cognitive reserve." Twenty-one studies which analyzed different CR proxies were selected and the level of evidence was classified a… Show more

“…Indeed, the only discernible clinical or cognitive factor separating these 2 groups of patients at baseline was premorbid IQ: the stable SZF group had a premorbid IQ comparable to healthy controls while the SZF→SZ group had a lower premorbid IQ. Our data point to the importance of cognitive reserve as a protective factor in staving off progression to schizophrenia 59 . A critical future step is to understand convergent changes in other structures known to be affected in schizophrenia, particularly the prefrontal cortex 60 .…”

“…53 A recent systematic review reported that an increased cognitive reserve, which is the ability to maintain skills developed throughout life, could be a protective factor in SZ and decrease symptom severity, being related to better functional outcomes. 99 To that end, cognition in SZ should not be viewed as a ''lost cause'' but a potential target for rehabilitation and remediation techniques. 100 Such interventions tailored for a specific cluster of patients could only be properly tested if we recognize the concept of different trajectories in SZ.…”

“…Therefore, we can affirm that CR is a multidimensional and multidisciplinary concept. Although research has mainly been carried out in the gerontological field for the prevention of dementia [ 21 ], it has generated a growing interest in psychiatry and psychopathology, applying CR in strokes [ 22 ], mood disorders [ 23 ] or schizophrenia [ 24 ].…”
