Inflammatory Pseudotumor in the Spiegel Lobe of the Liver of an Elderly Woman

Abstract: We describe an inflammatory tumor in the Spiegel lobe of the liver of an 81-year-old woman. The patient was referred to our hospital for evaluation of a fever of over 39 degrees C and upper abdominal pain. Both conditions had persisted for five days in spite of antibiotic treatment. Initial laboratory tests revealed a serum C-reactive protein concentration of 20.9 mg/dL and white blood cell count of 15,500/microL. Abdominal ultrasound showed a hypoechoic lesion measuring 4 cm in diameter in the Spiegel lobe of… Show more

“…There are reports of IPT showing early enhancement on CT with a washout phenomenon in the delayed phase, and hypervascularity on angiography, as in the present case. [15][16][17][18][19] Superparamagnetic iron oxide-enhanced MRI is a relatively new modality, which can be used for differentiating hepatic IPT from malignant tumors. Our PubMed search of the literature found only one other MRI study of IPT of the liver; however, in that case, Kupffer cell function was maintained within the lesion, 20 whereas in the present case this function was depleted.…”

“…Although multiple IPTs or IMTs of the liver are described in some reports, 14 , 21 , 31 – 33 they are mostly solitary lesions. 8 , 9 Both IPT and IMT of the liver are more likely to involve the right lobe of the liver, with rare cases reported in the Spiegel lobe and hilar region.…”
