Inflammatory pseudotumor associated with Mycobacterium tuberculosis infection

Abstract: The present case highlights the diagnostic importance of PCR-based detection of mycobacterial DNA in granulomatous tissue responses. It is of crucial importance that clinicians are aware of this unusual manifestation of mycobacterial infection to ensure that pertinent laboratory evaluation is employed and appropriate treatment is administered in order to avoid potential clinical implications.

“…Table 2 summarizes the main aspects in the differential diagnoses of IPTlike FDCT. In this context IPT may be also included, as it represents a lesion composed of ovoid to spindle cells admixed with polymorphic inflammatory cells, mostly T lymphocytes, but also B cells, plasma cells and histiocytes [16][17][18]. The nature of spindle cells in these lesions is still challenging because of their phenotypic variability.…”

“…These lesions are characterized by a clinical mass which is mostly identified either grossly or as an enhancing lesion on computed tomography (CT) scan. More than often, these masses pose features suggestive of malignancy, including ulceration and infiltration of adjacent structures [1][2][3]. Histologically, they reveal a dense chronic inflammatory infiltrate in a fibrous background and an abundance of plasma cells and/or histiocytes.…”

“…While the pathogenesis is not entirely known, many features of IPT can be related to production of inflammatory cytokines that sometimes can occur following surgery or even minor trauma [4][5][6][7][8]. Their occurrence is well-documented in the presence of another malignancy or secondary to multiple infectious agents [1,5,7,9]. We present a rare case of Mycobacterium avium complex (MAC) associated inflammatory pseudotumor of the nasal cavity in a patient with a history of intranasal cocaine abuse.…”

“…Various factors have been considered as potential originators of IMT, including infection, trauma and autoimmunity (5). Infection has been considered as a significant factor in the initiation of IMT, and several bacteria and viruses have been associated with the development of IMT, including Epstein-Barr virus, human herpes virus, Bacillus sphaericus and Mycobacterium tuberculosis (6). Recently, Li et al (7) described the case of a patient with IMT of kidney, who presented a long-term history of hepatitis B.…”
