Цель. Оценить диагностическую значимость биомаркеров при выявлении воспалительной кардиомиопатии (ВКМП). Материал и методы. В исследование включено 35 больных с предполагаемой воспалительной кардиомиопатией, хронической сердечной недостаточностью I-III функционального класса (ФК), сниженной систолической функцией левого желудочка (ФВ ЛЖ <40%). Всем пациентам наряду с клиническим обследова-нием проводились эндомиокардиальная биопсия (ЭМБ). При изучении биопта-тов использовались гистологический и иммуногистохимический методы с использованием моноклональных антител к моноцитам/макрофагам, к T-лим-фоцитам (CD 45RO, CD3+, CD4+, CD8+, CD68+). Диагноз ВКМП устанавливался, если в миокарде имелись воспалительные инфильтраты, связанные с дегенера-цией или некрозом миоцитов неишемического генеза, а количество лейкоцитов было 14 и более (включая до 4 моноцитов) в 1 мм 2 биоптата, не менее 7 из них должны были относиться к CD3+ Т-лимфоцитам. На основании результатов пациенты были разделены на группы ВКМП и невоспалительной дилатационной кардиомио патии (нвДКМП). У всех больных определялся уровень следующих биомаркеров: иммуноглобулины классов А, М, G, высокочувствительный С-реак-тивный белок (вчСРБ), матриксная металлопротеиназа-9 (ММП-9), трансформи-рующий фактор роста β (ТФРβ), фактор некроза опухоли α, (ФНОα), С3 и С4 компоненты комплемента, эозинофильный катионный протеин (ЭКП). Результаты. При сравнении групп ВКМП и нвДКМП по показателям гумораль-ного иммунитета было выявлено, что значения вчСРБ, С3 и С4 компонентов ком-племента, ММП-9, ТФР β были значимо выше в группе с ВКМП. При ROC анализе выявлено, что только вчСРБ, С3 и С4 компонентов комплемента и ММП-9 имеют диагностическое значение. Определены уровни указанных биомаркеров выше которых они имеют оптимальное соотношение чувствительности и специфично-сти: 1,13 мг/мл, 1,2 г/л, 0,256 г/л 680 нг/мл, соответственно. При использовании комбинации биомаркеров вчСРБ с С3, С4 компонентами комплемента чувстви-тельность составила 61%, специфичность 94%, при совместном применении ММП-9 и С3, С4 компонентов комплемента -66% и 94%, соответственно. Заключение. Комбинации вчСРБ, и ММП-9 с С3, С4 компонентами компле-мента целесообразно использовать для диагностики ВКМП. Aim. Background:Lymphocytic hypophysitis is a disorder of the pituitary gland that presents as a sellar mass lesion and/or hypopituitarism. It causes pituicyte destruction and hypopituitarism and is speculated to have an autoimmune basis. Diagnosis: Lymphocytic hypophysitis should be considered in the differential diagnosis of pituitary masses and/or hypopituitarism in females who are pregnant or in the early postpartum period, especially in cases associated with other autoimmune diseases or unusual patterns of hormone deficiencies. A definitive diagnosis requires tissue biopsy. A presumptive clinical diagnosis can be made based on a history of gestational or postpartum hypopituitarism, a contrast-enhancing sellar mass with imaging features characteristic of lymphocytic hypophysitis, a pattern of pituitary hormone deficiency with early loss of adrenocorticotrophic hormone and thyroid-stimulating hormone unlike that typically found with macroadenomas, relatively rapid development of hypopituitarism and a degree of pituitary failure disproportionate to the size of the mass.Symptoms resulting from partial or panhypopituitarism occur in approximately 80% of cases and multiple deficiencies are found in approximately 75% of cases. Management:Appropriate management remains controversial. Corticosteroid therapy has been advocated as a means of attenuating inflammation, but given the uncertainty of its efficacy and the known adverse effects, such therapy does not seem justified for most patients. The optimal surgical strategy involves partial resection of the mass to decompress the surrounding structures. All patients with lymphocytic hypophysitis require appropriate replacement therapy for deficient hormones. Long-term follow-up is mandatory to monitor for the development of other hormonal deficits.

Background: Nonischemic dilated cardiomyopathy (DCM) is associated with high mortality and morbidity. Cardiovascular magnetic resonance allows for the noninvasive assessment of function, morphology, and myocardial edema. Activation of inflammatory pathways may play an important role in the etiology of chronic DCM and may also be involved in the disease progression. Hypothesis: The purpose of our study was to assess the incidence of myocardial edema as a marker for myocardial inflammation in patients with nonischemic DCM. Methods: We examined 31 consecutive patients ( mean age, 57 ± 12 years) with idiopathic DCM. Results were compared with 39 controls matched for gender and age (mean age, 53 ± 13 years). Parameters of left ventricular function and volumes, and electrocardiogram-triggered, T2-weighted, fast spin echo triple inversion recovery sequences were applied in all patients and controls. Variables between patients and controls were compared using t tests for quantitative and χ 2 tests for categorical variables. Results: Ejection fraction (EF) was 40.3 ± 7.8% in patients and 62.6 ± 5.0% in controls (P < 0.0001). In T2-weighted images, patients with DCM had a significantly higher normalized global signal intensity ratio compared to controls (2.2 ± 0.6 and 1.8 ± 0.3, respectively, P = 0.0006), consistent with global myocardial edema. There was a significant but moderate negative correlation between signal intensity ratio in T2-weighted images and EF (−0.39, P < 0.001). Conclusions: Evidence shows that myocardial edema is associated with idiopathic nonischemic DCM. Further studies are needed to assess the clinical and prognostic impact of these findings.
