Inflammatory Cytokines and Matrix Metalloproteinases in the Synovial Fluid After Intra-articular Ankle Fracture

Abstract: In addition to anatomic fracture reduction, these data lend credence to reducing acute intra-articular inflammation through the development of antagonists to these pro-inflammatory and degrading mediators. Likewise, intra-articular lavage might reduce this inflammatory burden.

“…MMP13 is a collagenase with substrate specificity that targets collagen for degradation. Compared to other MMPs, MMP13 has a more restricted expression pattern in connective tissue (53). MMP13 has a much higher catalytic rate than other MMPs for ColII and gelatin, which makes it the most potent peptidolytic enzyme among collagenases (29).…”

“…Patients regain range of motion faster and start rehabilitation earlier than after open approaches 5 . Ankle synovial fluid after an intraarticular fracture has significantly elevated inflammatory cytokines and matrix metalloproteinases that may contribute to cartilage damage and synovitis 6 . One additional advantage to arthroscopy is that lavage may theoretically reduce these factors that may be worsening outcomes of otherwise appropriate reductions 7 …”

“…One could speculate that a self-adjusting mechanism for IL-10 by an autocrine inhibition takes place in fibro-chondrocytes as well and that this is responsible for less pronounced effects with higher IL-10 dosages. Anyway, the concentration of IL-10 in a healthy joint or even after injury has been found to be lot lower than 10 ng/ml [37, 38]. However, at this moment in time a wide range of thematically related studies, that directly applied IL-10, used dosages ranging prom 10 - 300 ng/ml [22, 3941].…”
