Background Inflammatory bowel disease (IBD) and its main subtypes, including Crohn's disease (CD) and ulcerative colitis (UC), have been suggested to be linked with chronic obstructive pulmonary disease (COPD) progression, but the potential bidirectional causal association between them remains unclear. Methods We drew on summary statistics from the large-scale genome-wide association studies and adopted the inverse variance weighting (IVW)method as the main mendelian randomization analysis method to estimate the causal effect. Results There was no evidence support that COPD was associated with increased risk of IBD and two main subtypes (UC and CD) (IBD: odds ratio (OR) [95% confidence interval (CI)] = 0.99[0.90–1.09], P = 8.63×10− 1; UC: OR [95% CI] = 0.96[0.86–1.07], P = 4.70×10− 1; CD: OR [95% CI] = 1.10[0.89–1.35], P = 3.81×10− 1;). The IVW method provided evidence for a negative causal effect of IBD and two main subtypes (UC and CD) on COPD (IBD: OR [95% CI] = 0.93[0.89–0.97], P = 4.86×10− 4; UC: OR [95% CI] = 0.94[0.90–0.98], P = 6.57×10− 3; CD: OR [95% CI] = 0.96[0.94–0.99], P = 4.92×10− 3). The results of the sensitivity analyses were robust and no evidence of heterogeneity and horizontal pleiotropy was observed. Conclusions The present study showed that COPD does not affect IBD, but IBD and its main subtypes (UC and CD) have a negative causal effect on COPD. And understanding the confounding factors that affect clinical research will help the clinical treatment and management of the two diseases.

Inflammatory bowel disease (IBD) is one of the intractable diseases. Nutritional components associated with IBD have been identified, and it is known that excessive methionine intake exacerbates inflammation, and that tryptophan metabolism is involved in inflammation. Analysis of the gut microbiota has also progressed, where Lactobacillus regulate immune cells in the intestine and suppress inflammation. However, whether the methionine and tryptophan metabolic pathways affect the growth of intestinal Lactobacillus is unknown. Here we show how transient methionine, tryptophan, and niacin deficiency affects the host and gut microbiota in mouse models of colitis (induced by dextran sodium sulfate) fed a methionine‐deficient diet (1K), tryptophan and niacin‐deficient diet (2K), or methionine, tryptophan, and niacin‐deficient diet (3K). These diets induced body weight decrease and 16S rRNA analysis of mouse feces revealed the alterations in the gut microbiota, leading to a dramatic increase in the proportion of Lactobacillus in mice. Intestinal RNA sequencing data confirmed that the expression of several serine proteases and fat‐metabolizing enzymes were elevated in mice fed with methionine, tryptophan, and niacin (MTN) deficient diet. In addition, one‐carbon metabolism and peroxisome proliferator‐activated receptor (PPAR) pathway activation were also induced with MTN deficiency. Furthermore, changes in the expression of various immune‐related cytokines were observed. These results indicate that methionine, tryptophan, and niacin metabolisms are important for the composition of intestinal bacteria and host immunity. Taken together, MTN deficiencies may serve as a Great Reset of gut microbiota and host gene expression to return to good health.

Risk Factors for Early Postoperative Complications in Acute Colitis in the Era of Biologic Therapy

BACKGROUND: Despite major advances in the clinical treatment of inflammatory bowel disease, some patients still present with acute colitis and require emergency surgery. AIMS: To evaluate the risk factors for early postoperative complications in patients undergoing surgery for acute colitis in the era of biologic therapy. METHODS: Patients with inflammatory bowel disease admitted for acute colitis who underwent total colectomy at a single tertiary hospital from 2012 to 2022 were evaluated. Postoperative complications were graded according to Clavien-Dindo classification (CDC). Patients with more severe complications (CDC≥2) were compared with those with less severe complications (CDC<2). RESULTS: A total of 46 patients underwent surgery. The indications were: failure of clinical treatment (n=34), patients’ or surgeon's preference (n=5), hemorrhage (n=3), toxic megacolon (n=2), and bowel perforation (n=2). There were eight reoperations, 60.9% of postoperative complications classified as CDC≥2, and three deaths. In univariate analyses, preoperative antibiotics use, ulcerative colitis diagnosis, lower albumin levels at admission, and preoperative hospital stay longer than seven days were associated with more severe postoperative complications. CONCLUSIONS: Emergency surgery for acute colitis was associated with a high incidence of postoperative complications. Preoperative use of antibiotics, ulcerative colitis, lower albumin levels at admission, and delaying surgery for more than seven days were associated with more severe early postoperative complications. The use of biologics was not associated with worse outcomes.
