Inflammatory bowel disease with primary sclerosing cholangitis: A Danish population‐based cohort study 1977‐2011

Abstract: This unselected population-based study shows that PSC-IBD patients not only have an extensive phenotype of IBD, they are also treated more intensively than other patients with IBD. However, the prognosis remains poor and without any apparent improvement over calendar time.

“…Approximately 70% of patients with PSC have or will develop concurrent IBD, of which the majority, about 80%, will suffer from ulcerative colitis (UC). [2][3][4] Conversely, the presence of PSC among the total population of patients with IBD is rare, with a prevalence varying between 1.7-3.8%. 5,6 It has been proposed that IBD in PSC represents a distinct phenotype, characterised by a higher frequency of pancolitis with involvement of the right colon and a relatively mild disease course, and a higher occurrence of pouchitis after colectomy, compared to IBD without PSC.…”

“…Up to 50% of these cases are recorded in the first year of diagnosis of PSC (10). The presence of chronic inflammation of bile ducts typically associated with PSC is thought to be one of the reasons for this heightened risk (11)(12)(13). Other factors that serve as the link between PSC and CCA include increased proliferation of the epithelial cells of the biliary tree, cholestasis in the ducts leading to the liver and presence of mutagens produced in the bile (10,11).…”

The role of inflammation in the growth and rapid development of CCA is also underscored by studies that identify inflammatory bowel disease (IBD) as one of the risk factors of CCA (12,13).…”

“…В большинстве случаев (72%) это были пациенты с ЯК, и у 78% из них отмечалось тотальное поражение. У 91% пациентов с ПСХ и болезнью Крона отмечалось поражение толстой кишки [3]. Анкилозирующий спондилит при ВЗК чаще встречается у мужчин и чаще при болезни Крона (2,7%) [1].…”
