La quimioterapia sistémica destruye las células malignas con alto recambio celular y altera de forma indirecta a otros tejidos como la piel. Las queratosis actínicas son lesiones premalignas con alto recambio que pueden inflamarse de manera indirecta con la quimioterapia sistémica. Objetivo: Identificar las características clínicas y epidemiológicas de los pacientes con inflamación de queratosis actínicas secundaria al uso de quimioterapia y mostrar las opciones terapéuticas. Material y métodos: Se realizó una búsqueda de casos en PUBMED, LILACS y SciELO con los términos "queratosis actínicas" y "quimioterapia". Se recolectó la información epidemiológica y se elaboró una tabla con la información más relevante. Los datos se analizaron con el programa Epidat 4.2. Resultados: Se identificaron 21 casos con administración de quimioterapia sistémica e inflamación de queratosis actínicas. La media de edad de los pacientes fue de 67.9 años y 42.9% de casos en mujeres. El tiempo promedio de aparición de las lesiones fue de 11.5 días. Las lesiones ocurrieron en sitios fotoexpuestos e iniciaron como un exantema pruriginoso. Doce casos se trataron con esteroides tópicos y experimentaron mejoría. Conclusiones: La inflamación de las queratosis actínicas por quimioterapia sistémica es un efecto adverso que requiere una atención con medidas generales mínimas y prescripción de antiinflamatorios tópicos con resolución de las lesiones. En ningún momento es necesaria la suspensión del tratamiento de la anomalía subyacente. Es importante reconocer este cuadro para tranquilizar al paciente, instituir tratamiento oportuno y mejorar su calidad de vida.
