Inflammation and l-carnitine therapy in hemodialysis patients: a review

Abstract: Inflammation is a common complication in hemodialysis (HD) patients with no valid treatment strategy. In addition, carnitine deficiency occurs frequently in HD patients because of intradialytic loss of carnitine, impaired de novo carnitine renal synthesis, and reduced dietary intake. It appears that carnitine deficiency is related to inflammation in HD patients. A few clinical trials have investigated the effect of L-carnitine supplement on inflammatory markers in HD patients. All studies in this field, except… Show more

“…Intestinal inflammation and microbiome disturbances may play a major role in chronic inflammation and in the consequences of adverse chronic kidney disease. A high concentration of serum inflammation markers is an important risk factor for CVD in hemodialysis patients . Several studies have been conducted to find treatment strategies for reducing inflammation in hemodialysis patients; however, no valid treatment has been found to‐date .…”

“…Probiotics are nonpathogenic live microorganisms given orally that establish residence in the gut; their presence is postulated to have beneficial effects on the host's health. Some of these suggested beneficial effects include: improving indigestion and lactose intolerance, cancer suppression, antimicrobial effects and immune system induction . Prebiotics, in distinction to probiotics, are indigestible food ingredients which are given in an attempt to stimulate growth and/or activity of beneficial gut bacteria.…”

“…Karnitin (hidroksi N trimetil aminobütirik asit) hayvansal kaynaklı besinlerin çoğunda bulunmaktadır ancak bitkisel kaynaklı besinlerde sınırlıdır (67). Karnitin ve inflamasyon arasındaki bağlantıya dair mekanizmalar net olmamasına karşın, karnitinin fazla ve toksik olan açil grupların vücuttan uzaklaştırılmasında rol alması ve serbest radikalleri süpürücü etkisinin olması; anti-inflamatuvar aktivitesinin altında yatan mekanizmalar olabileceği düşünülmektedir (68,69).…”
