8Рак желудка (РЖ) остается одной из наиболее частых причин смерти от злокачественных опухолей во всем мире [1,2]. Определение факторов, влияющих на прогноз РЖ, важно как для диагностики, так и для поиска новых схем лечения этого заболевания. Одним из таких факто-ров является выраженность воспалительных изменений в опухоли и прилежащих тканях. Однако оценка его роли в опухолевой прогрессии неоднозначна. Одни авторы счи- Цель исследования -изучение особенностей перитуморальной инфильтрации слизистой оболочки желудка (СОЖ) В-лимфоцитами и их связь с клиническими характеристиками рака желудка (РЖ). Материал и методы. Исследованы об-разцы ткани опухоли и прилежащей СОЖ у 73 радикально оперированных пациентов РЖ. Гистопрепараты окрашивали гематоксилином и эозином и иммуногистохимически, с использованием антител к CD20. Результаты. В перитумораль-ной области отмечено три вида структур с экспрессией CD20: В-лимфоциты, расположенные диффузно, лимфоидные фолликулы (ЛФ) и очаговые лимфоидные инфильтраты (ЛИ). Установлены отрицательные корреляции плотности СD20 В-лимфоцитов с распространенностью РЖ. Наиболее низкие значения плотности СD20 В-лимфоцитов наблюдались при низкодифференцированной аденокарциноме и недифференцированном РЖ (р=0,051), при глубине инвазии опухоли Т1-2 (р=0,06) и наличии метастазов в регионарных лимфатических узлах (р=0,02). Наличие ЛИ и ЛФ было связано толь-ко с гистологическим типом РЖ. При диффузном и кишечном типах РЖ очаговые ЛИ наблюдались в 46,7 и 21,4% случаев (р=0,15) и множественные ЛФ -в 72,7 и 38,9% случаев (р=0,07) соответственно. Наличие очаговых ЛИ было связано со статистически недостоверным снижением 3-летней общей выживаемости с 94,7 до 70,0% (р=0,07), а наличие множе-ственных ЛФ -со снижением 3-летней безрецидивной выживаемости с 75 до 40% (р=0,08). Заключение. Полученные данные свидетельствуют о связи В-лимфоцитов с клиническими характеристиками и отдаленными результатами лечения РЖ. Полагаем, что необходимы дальнейшие исследования роли В-лимфоцитов в прогрессии РЖ. Material and methods. Tumor and adjacent GM tissue samples from 73 radically operated patients with GC were investigated. Histological specimens were immunohistochemically stained with hematoxylin and eosin, by applying anti-CD20 antibodies. Results. The peritumoral area showed three types of CD20 expression structures: diffusely located B lymphocytes; lymphoid follicles (LFs); and focal lymphoid infiltrates (LIs). Degradation of the stromal connective tissue and basement membrane components are key elements in tumor invasion and metastasis. Some components, particularly the interstitial collagens, are very resistant to proteolytic attacks and can be degraded by specific proteinases like Matrix Metalloproteinases (MMPs). MMPs can also impact on tumor cell behavior in vivo as a consequence of their ability to cleave growth factors, cell surface receptors, cell adhesion molecules, or chemokines/cytokines, and for stimulating angiogenesis. Different molecular expression profiles of MMPs and their inhibitors (TIMPs) have been associated with the main steps in breast cancer progression, such as creating a potential invasive phenotype in Ductal Carcinoma in situ (DCIS), favoring the hematogenous dissemination, and enabling the metastatic progression across the axillary lymphatic system. These associations have clinical interests, as they can contribute to a better characterization of early breast carcinomas (which differ in their both biological and clinical behavior), evaluate microinvasion in resection specimens of breast tumors, provide a more precise prognostic, and predict the tumoral status of non-sentinel lymph nodes in breast cancer. It is also especially remarkable the evidences indicating that MMPs and TIMPs expression in individual cell populations from tumor stroma, such as mononuclear inflammatory cells (MICs) and fibroblast, clearly impacts on the clinical outcome of breast cancer patients. There are several factors linking inflammation, MMP activity and breast cancer. This knowledge will be useful to develop novel therapies and prevention strategies targeting critical components.
