Infective Endocarditis in Childhood: a SingleCenter Experience of 18 Years

Abstract: Introduction We aimed to present the risk factors, clinical and laboratory findings, treatment management, and risk factors for morbidity and mortality of infective endocarditis (IE) as well as to relate experiences at our center. Method We retrospectively analyzed data of 47 episodes in 45 patients diagnosed with definite/possible IE according to the modified Duke criteria between May 2000 and March 2018. “…Fever was found to be the most common symptom in our cohort which was reported in more than 90% of patients which is similar to that reported by Assiri [12], as well as to that reported in most patients presenting to the emergency department with IE [17], however, fever was reported slightly less by Zaqout et al in Qatar (84%). Only one-fifth of our patients had a documented audible murmur which is much lower than a study by Yakut et al that reported a new murmur or changing murmur in 68% of patients [18]. Previous studies from KSA and Qatar did not report the number of murmurs in their cases.…”

“…Regarding vegetations detected by echocardiography among our patients, vegetations were present in 77 (77%) patients. A nearly equal percentage was detected in Egyptian adults 76.9% (17) , yet higher ratios were reported by other studies in Africa 84.9% and 95.5%, respectively (5,10) .…”

“…The occurrence of IE in children is perhaps increasing partly due to the increased incidence of IE inpatients with congenitally malformed hearts who are now surviving longer and partly due to increased use of central venous catheters in intensive care units (5) .…”

“…The studies conducted in Turkey have reported that the most common risk factor is CHDs, and 43-91% of children with IE have underlying CHDs. Various rates between 0% and 43% have been reported for rheumatic heart disease [ 14 - 18 ]. Our results support that CHDs are the first predisposing factor; however, rheumatic heart disease is still an important risk factor in our country.…”
