Infectious Triggers in Periodontitis and the Gut in Rheumatoid Arthritis (RA): A Complex Story About Association and Causality

Abstract: Rheumatoid arthritis (RA) is a systemic immune mediated inflammatory disease of unknown origin, which is predominantly affecting the joints. Antibodies against citrullinated peptides are a rather specific immunological hallmark of this heterogeneous entity. Furthermore, certain sequences of the third hypervariable region of human leukocyte antigen (HLA)-DR class II major histocompatibility (MHC) molecules, the so called "shared epitope" sequences, appear to promote autoantibody positive types of RA. However, M… Show more

“…Several genetic, environmental, and socioeconomic factors are associated with RA initiation and progression [ 44 ]. Fat-rich and low-vegetable diet, smoking, gut microbiota, periodontal diseases, bacterial or viral infections lead to different inflammatory conditions and results in RA [ 47 , 48 , 49 ] ( Figure 1 ). RA is also influenced by gender as women are more susceptible to RA than men and several factors influence the disease, such as hormonal deficiency (estrogen deficiency in pre- and post-menopausal women), genetic factor (X-chromosome related), the prevalence of depression, neuropathic pain, fibromyalgia and osteoporosis (in post-menopausal women) [ 50 , 51 , 52 , 53 ].…”

“…Prevotella is also present in the oral cavity, and their presence increases in periodontal diseases, and thus RA is also correlated with periodontitis [ 49 , 101 , 102 ]. Periodontitis is highly correlated with RA in the clinic, and the oral bacterial population shift towards Porphyromonas gingivalis and other anaerobic bacteria such as Prevotella spp.…”

“…A chief suspect among the environmental factors that influence arthritis are periodontal pathogens [ 3 ]. Periodontal Disease (PD) is a widespread pathology characterized by chronic inflammation in the oral mucosa that can result in loss of tooth function and damage to the underlying alveolar bone structure.…”
