Infectious Complications of Acute and Chronic Liver Disease

Abstract: Acute and chronic liver diseases are frequently complicated by infections, which result in increased morbidity and mortality and place an economic burden on health care systems. This review discusses the epidemiology and the impact on prognosis of infections in liver cirrhosis, nonalcoholic fatty liver disease/nonalcoholic steatohepatitis, acute liver failure, and post-liver transplantation. Possible mechanisms for this increased susceptibility are innate immune dysfunction (Kupffer cells, neutrophils, monocyt… Show more

“…For this reason, infection is considered an important prognostic marker in patients with ESLD. 7 Bloodstream infections (BSIs) are a common complication in patients with ESLD, affecting 4-21% of patients, 8 making them 10 times more common in cirrhotic than in non-cirrhotic patients. 9 The increased gut permeability and the cirrhosis associated immune dysfunction (CAID) make this population prone to develop BSI by endogenous route, particularly during acute-on-chronic liver failure (ACLF).…”

“…The reasons for the susceptibility to infections associated with bacteremia are different and include impaired immunity, bacterial translocation, and clinical factors (i.e., variceal bleeding, spontaneous bacteria peritonitis, skin lesions) [12]. Healthcare-associated infections are reported with increasing frequency in this setting; about 15-35 % of patients with liver disease develop nosocomial infections during their hospital stay as compared with 5-7 % in the general hospital population [13]. Cirrhosis has been associated with an increased risk of developing an episode of IE in a cohort of patients followed for three years [2].…”
